What causes fever, shivering, and throat pain after minor injury?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

Three days ago, I played an outdoor sport and sustained several minor injuries. The next day, I experienced fatigue and body aches. By afternoon, the fever started, followed by shivering in the evening, although it was not very cold.

The next day, the shivering stopped, but then I started having throat pain on one side, the left side above the vocal cord, which becomes more intense when swallowing saliva, drinking water, or eating.

Today (the fourth day), I started having a headache when making sudden movements, as if something was fluidly moving inside my head a few seconds after the movement. I also have a dry mouth and lips. Please advise on diagnosis, treatment, and home remedies for my case.

Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern.

I am able to determine a few differential diagnoses based on your symptoms and history. They are:

  1. Acute tonsillitis.

  2. Rhabdomyolysis.

  3. Acute sinusitis.

  4. Viral fever.

Rigorous exertion during a sport can lead to breakdown of muscle, which can cause fever and shivering. Seasonal infections can lead to upper respiratory tract symptoms or high-grade fever. Travelling and being exposed to different organisms can lead to viral and bacterial infections, especially during winter and rainy.

I would like to know whether the injuries you sustained included a head injury as well.

  1. Have you noticed any changes in your urine lately?

  2. Is there any cough with sputum or a running nose?

  3. I would also like to know whether you can see any white layers in your throat or on your tongue. Please mention your highest temperature recorded with a thermometer.

  4. Do you have any non-chronic illnesses?

  5. Any family history of chronic disease?

I suggest you undergo these investigations:

  1. Hemogram.

  2. Liver function test.

  3. Kidney function test.

  4. Urine routine.

  5. CT (computed tomography) scan of the head with contrast.

For the moment, take Paracetamol 650 mg and rest at home for three to five days. Kindly get back to me once your reports are available.

I hope that this answers your query.

Kindly follow up if you have more doubts.

Thank you.
