Hi doctor,

I had to take the oral contraceptive pills from the second day of my cycle. Due to some issues, I could not start my medicine. My last period was on the fifth day of the last month. I have been having light bleeding and brown/black discharge since the twenty-first day of the last month. I want to start taking the pills as prescribed by you from the second day but I still have not got my periods properly. I am just having that blackish discharge now. Should I wait for my periods to start properly or should I start with the pills course from today itself? Please guide me. Also, in the second last month, there was no intermenstrual bleeding and in the last month, it started again with a lot of acne on my face.

The intermenstrual bleeding being experienced is due to anovulatory cycle in the last month. As long as the bleeding would continue, fresh menstruation would not start as you are experiencing shedding of endometrium already. You can start taking the pills from today or tomorrow for 21 days, and post-21 days the menstruation should be expected in the next three to five days.

You should take the pills at a fixed time daily (say 8 to 10 PM). They can be taken an hour after dinner. You can start from one end of the pack, ignore the days and start from tomorrow.

After finishing the pack of 21 tablets, wait for your menstruation to start, and then from day two of the next cycle, start a fresh 21 days pill pack for the second cycle.

I have not been feeling well since yesterday. I have a sore throat and fever. Can I take medicines like Crocin or Azee along with OCP?

I suggest you take-

1. Azee (Azithromycin) 500 mg once a day.

2. Crocin (Paracetamol and caffeine) twice a day.

3. Do warm saline water gargles twice a day.

I also suggest you consult your specialist doctor, discuss with them and take medicines with their consent.

This is my second cycle of oral contraceptive pills. During the last cycle, I took Mypill, and in this cycle, I am taking Ginnette 35. I did not have any spotting or intermenstrual bleeding during my last cycle. Yesterday, was my seventh day of the cycle, and I got my period on the second of this month. My period usually lasts for seven days. Today is the eighth day, but I still have brown and black discharge. Is this normal? What could be the reason for this discharge?

If you had started Ginette 35 (Cyproterone and Ethinyl Estradiol) from day three of your fresh cycle at a fixed time, then this brownish blackish discharge would not have occurred.

I suggest you keep yourself well hydrated while you are taking the tablet, Ginette.

I hope you are also taking tablet Ovabless-Myo (Myo- Inositol, D- Chiro- Inositol, Chromium, and Vitamin D3) along with it.

Continue Ginette, and bleeding shall stop on its own.

I hope you have lost weight as well.

There is no apparent need to be worried if it is just spotting.

I suggest you take tablet Trenaxa MF (Tranexamic acid and Mefenamic acid) twice daily for three days.

It will help to stop this discharge as this is an antifibrinolytic.

I also suggest you continue medications as advised for a total of three cycles, and post that, ultrasonography would be required to check the status of PCOD (polycystic ovary syndrome).

If the blood was post intercourse, then there is a likelihood of cervicitis, inflammation of the cervix, or cervical erosions. These conditions can cause bleeding post intercourse due to frictional injury.

Also, burning urination is an indication of possible urinary tract infection (UTI) which shall need antibiotic treatment.
