Hello doctor,
My brother has low hemoglobin, which is 4.7 g/dL and has severe iron deficiency. His first blood transfusion was 10 years ago, and he recently had his second transfusion. He also has an intellectual disability due to Down syndrome. I want to know why this is happening and what the treatment options are.

Iron deficiency anemia (IDA) occurs when there is a dietary deficiency of iron, increased blood loss, increased physiological demand, malabsorption, or other factors. In young individuals, the demand for iron increases, and if the person has mental retardation, they may not consume a balanced diet, especially iron-rich foods, which can lead to anemia. Please provide any reports or tests that have been done to confirm the diagnosis of iron deficiency anemia (IDA), if possible.
For severe cases of iron deficiency anemia, where hemoglobin levels are less than 7 gm/dl, intravenous iron or blood transfusion may be required. After stabilization, iron tablets should be prescribed to replenish iron levels. Recommend an iron-rich diet including green leafy vegetables, jaggery, dried fruits, beans, and red meat, which can help increase iron intake. If a specific cause, apart from dietary intake, is found, treatment should be adjusted accordingly. For instance, if abdominal pain is present and stool tests show occult blood and parasite eggs, then Albendazole should be prescribed for treating intestinal parasites.
