How to manage frequent urges but only small stools in IBS?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am 43 years old and passing stools five to seven times daily. I did a scan, which showed that my kidneys are normal, and in conclusion, the scan pointed out IBS. However, I am now feeling fullness, and each time I stool, only a small amount comes out, even when I have the urge to go. I am still passing small stools.

I am taking Flora Nor and Vitamin C 1000 mg. I had similar issues before, but after treatment, they stopped. This time, it has not stopped.

Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern.

Your symptoms fit well with irritable bowel syndrome (IBS), a functional bowel condition in which the intestine becomes over-sensitive and poorly coordinated. This causes frequent urges to pass stool but only small amounts each time, along with a feeling of fullness or incomplete evacuation, even though scans and kidneys are normal.

Vitamin C at 1000 mg can worsen loose stools and urgency, and should be stopped for now. Probiotics may help, but usually need time and are not sufficient alone.

Home management is very important in IBS and focuses on calming the gut. I suggest you follow these instructions mentioned below:

  1. Eat meals at regular times.
  2. Avoid overeating.
  3. Reduce foods that commonly trigger symptoms, such as spicy and oily foods, caffeine, alcohol, fizzy drinks, excess sugar, artificial sweeteners, and sometimes milk if it worsens symptoms.
  4. Increase soluble fibre slowly using foods like oats, bananas, rice, apples without peel, or a small amount of isabgol at night with warm water, increasing gradually to avoid bloating.
  5. Drink adequate water, avoid very cold drinks, and include simple remedies such as peppermint or fennel tea for gas and bowel spasm.
  6. Daily walking or light exercise helps regulate bowel movements, and stress control through relaxation, prayer, or breathing exercises is essential because stress often worsens IBS even if you do not feel anxious.

IBS improves gradually, not suddenly. However, if you develop warning signs such as weight loss, blood in stool, fever, anemia, or symptoms that wake you from sleep, you should see a doctor.

Let me ask you this: do you notice your symptoms getting worse after specific foods or during stressful periods, or are they present all the time regardless of what you eat?

I hope that this answers your query.

Kindly follow up if you have more doubts.

Thank you.
