What should I take for IBS-M if prebiotics don't suit me?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have been suffering from IBS-M and experiencing five to six motions a day.

I am taking some medicines for migraines, also Magrium one tablet daily, Inderal, and a Fludac capsule daily. I am lacto-intolerant, and probiotics do not suit me at all.

Kindly prescribe some supplements or vitamins because prebiotics do not suit me.

Thanks.

Hello,

Welcome to icliniq.com.

I can understand your concern.

Given your long-standing mixed IBS (irritable bowel syndrome) with frequent, unsatisfactory bowel movements and your intolerance to prebiotics and probiotics, especially since they worsen your migraines, it is important to approach management cautiously.

Since your stools are neither too loose nor too hard, but you have frequent bowel motions with incomplete evacuation, this suggests a possible component of altered bowel motility and visceral hypersensitivity.

I would advise avoiding prebiotics and probiotics, as you have noted intolerance. Instead, consider a low FODMAP (fermentable oligosaccharides, disaccharides, monosaccharides, and polyols) diet trial under dietitian guidance, which often helps reduce IBS symptoms without triggering migraines.

For symptom relief, medications like low-dose antispasmodics (for example, Hyoscine butylbromide) can help reduce bowel spasms and discomfort.

Additionally, since you have migraines and take medications like Inderal (Propranolol), we need to be mindful of drug interactions and side effects. Fiber supplementation with soluble fiber (such as psyllium husk) may help normalize bowel movements without worsening symptoms, but start at a low dose to avoid bloating.

Vitamin supplements are not specifically indicated for IBS unless there is a documented deficiency. If symptoms persist, a referral for further evaluation, including colonoscopy or motility studies, may be needed to rule out other causes. Stress management and regular exercise can also improve symptoms.

I hope this information will help you.

Revert in case you have any doubts.

Thanks.
