Why does stress trigger IBS flares despite a clean diet at 28?

Hello doctor,

I am 28 and have been managing IBS for about three years now, and I follow a strict low-FODMAP diet religiously. But every single time I go through a stressful period at work, my IBS symptoms come back with full force, cramping, bloating, urgent loose stools, the whole thing.

My gastroenterologist ran a full panel: calprotectin came back at 62, and celiac antibodies were negative. He says it’s classic IBS, but I do not understand why the gut reacts so badly to stress when I am doing everything right with the diet. My GP prescribed Mebeverine, but it barely helps during these episodes.

I have also started noticing that my IBS flares coincide with tension headaches and jaw clenching. Is there a gut-brain connection that is making stress override my clean diet? What else can be done beyond just dietary control for IBS at 28?

Your test results are supportive of typical irritable bowel syndrome, as a calprotectin level of 62 is only mildly elevated and not usually suggestive of major inflammatory bowel disease, while negative celiac antibodies make celiac disease unlikely.

Mebeverine may help reduce bowel spasms, but it often does not fully control stress-related IBS symptoms because it does not address the nervous system component.

In many patients, IBS symptoms improve when stress management strategies such as regular sleep, relaxation exercises, mindfulness, physical activity, or psychological therapies are combined with dietary measures. Keeping a symptom diary may also help identify specific stressors or food triggers that worsen symptoms.
