Hello doctor,

I have had leucorrhea for 17 years. After getting married, this issue has worsened. I have taken numerous antibiotics to treat leucorrhea, but over time, become more prolonged. Additionally, I have developed other health concerns. For the past year, I have been dealing with prolonged bleeding during my menstrual cycle. The bleeding extended beyond the expected duration, and it was too heavy bleeding with blood colts. I recently took Primolut N last month. I used to take Primolut-N for 21 days before also, to regulate my periods. However, after taking the medication, my bleeding only lasted for six days. After that, the prolonged bleeding recurs approximately every four months and does not seem to stop. And whenever I had sex, I had bleeding spots.

I read your query and can understand your concern.

Your problem has not been properly addressed, so it is getting worse day by day. Discharge before marriage is normal, as it is usually physiological secretions from the body of the uterus and cervix.

But after marriage, the infection from semen enters the vagina and cervical canal. It ascends into the uterus, and then into the tubes and the sides of the uterus, and the surrounding structures including muscles and ligaments.

So, the clear or whitish discharge, before marriage may become yellowish, greenish, grayish, or dirty brownish, with or without foul smell and itching. Other symptoms may include a throbbing sensation in the lower abdomen before periods, mild fever during periods, and pain that becomes severe during periods and at sex, especially at the time of orgasm, when traction occurs on the surrounding muscles and ligaments. Pain radiating to thighs, low back, and bone above the vaginal opening. Sex and periods followed by excessive discharge for a few more days.

There will be raw areas over the cervix, which remain raw, and each time when semen or the penis touches it, you get fresh red or pink blood with a lot of discharge.

This infection gradually goes into the uterus and increases the thickness of the endometrium (inner lining of the uterus), so the thickness is neither uniform nor healthy. This layer keeps peeling off as thick, brownish, or purple-colored clots or fleshy masses, which keep on falling as the whole thickness has not gone.

You need to treat it, as it can go higher and affect the tubes. It will damage sperm and reduce your future chances of fertility.

Do not worry, there is a solution. If you are currently experiencing bleeding;

Take Tablet Primolut-N (Norethisterone 5 mg) two tablets thrice till the bleeding stops fully (may take one to two days), then two tablets twice a day for two days, and then 1 tablet twice a day for 18 days.

You will have thick, dark periods (long fleshy mass) within seven to 10 days of the last pill. Do not forget pills in between. This will remove the whole thickness in one piece this is called medical D and C (dilation and curettage).

To address infection, both you and your partner have to take medicines simultaneously. If only one is treated, the other will reinfect him or her when you have sex again.

You and your partner can have –

1. Tablet Fluconazole 150 mg (antifungal) stat (only one tablet, no repetition) after lunch.

2. Tablet Azithromycin 1 gm stat (only one tablet, no repetition) one hour before food.

3. Tablet Secnidazole forte two tablets stat (no repetition) after dinner.

Take all these on the same day by both. Additionally, you should have a capsule Doxycycline 100 mg twice a day for one week, perform vaginal wash with V WASH or a similar cleanser twice a day, and vaginal pessary such as a cap. Clingen forte or Candid -CL or Cansoft CL once at night for five nights.

Your partner can have local application of any antibiotic and antifungal cream such as Candid B or Candiderma or Lobate GM on the penis twice a day after washing for seven days.

Do not have sex till the infection is fully gone. Do treat it, as delaying treatment may allow the infection to progress inward and may affect both you and your partner's future fertility. I am sure you will be fine, will not any medications for periods, and will be relieved of all these complaints within two to three weeks
