Hello doctor,

A recent sonography of a liver cirrhosis patient revealed that his liver is enlarged to 7.1 inches and has signs of liver parenchyma. His platelets depleted to 75000, has abdominal pain, and cannot sit. He is sleeping often and has a high blood pressure of 150/120 these days. He also has severe chest pain. He also has constipation and no motions for four or five days despite using looz syrup and anema. Due to the coronavirus outbreak, we are not able to go to a hospital, as doctors have shut down the OPD and are not even taking in patients. Kindly suggest getting relief from chest, and abdominal pain, BP, and motions.

I have read your query and understand your concern. I suggest you take the following-

Give tablet Dulcolax (Magnesium stearate and Lactose)10 mg- two tablets at a time for motion with syrup Cremaffin plus (Sodium Pico-sulfate and Liquid Paraffin) two tablespoons. You can add if it is still not happening.

For pain, I think it is because of constipation, but you can use the tablet Ultracet (Tramadol and Acetaminophen) 50 mg two times a day for pain after food.

You can give him Hepamerz (L-Ornithine-L-Aspartate and Pancreatin) sachet thrice a day till you feel he is sleepy.

Thank you for the reply.

The patient has had severe mental confusion for two to three days. What actually is happening to him? Kindly suggest medicine to treat this. Also, why is such confusion happening?

It is because of hepatic encephalopathy, a common condition associated with liver disease. Ensure adequate stool passage a minimum of three to four times a day. Give a Hepamerz sachet of five grams four times a day and dissolve in water, it is available in pharma. Avoid all sleeping pills and Aldactone (Spironolactone) and Lasix (Furosemide) if he is already on.
