What causes painful lumps in the genital area?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a female experiencing painful, swollen lumps on both outer labia. Additionally, I have smaller painful lumps on either side of my bikini area and a large purpura (bruise-like discoloration) on my thigh. I am not currently taking any medication. I would like help in understanding the potential causes or what steps to take next?

Kindly suggest.

Thank you.

Hello,

Welcome to icliniq.com.

I understand your concern.

Painful swollen lumps on the outer labia and bikini area, along with a large purpura (bruise-like lesion) on your thigh, could indicate conditions such as Bartholin’s cyst, folliculitis, hidradenitis suppurativa, or even an underlying autoimmune or blood disorder. Bartholin’s cysts occur when the gland becomes blocked, potentially leading to an abscess, while infected hair follicles (folliculitis) can cause painful lumps, especially after shaving or sweating. Hidradenitis suppurativa is a chronic inflammatory skin condition that causes recurrent painful nodules in the groin area. The presence of purpura without trauma might suggest a blood clotting disorder or vasculitis, particularly if accompanied by fatigue, fever, or unexplained bruising elsewhere. Additionally, if sexually active, sexually transmitted infections (STIs) like herpes or syphilis should also be considered, especially if there are genital ulcers or unusual discharge.

To manage your symptoms, applying warm compresses may help with drainage if the lumps are infected, and wearing loose cotton underwear can prevent further irritation. Avoid shaving the affected area and keep it clean and dry. However, if the lumps continue growing, become severely painful, or are accompanied by fever, chills, or flu-like symptoms, seek medical attention promptly. The purpura should also be monitored closely if it spreads, darkens, or new bruises appear, an urgent medical evaluation is necessary. Given your worsening symptoms, consulting a gynecologist, dermatologist, or general physician would be advisable to determine the underlying cause and receive appropriate treatment. Would you like guidance on what tests may be needed?

I hope this helps.

Let me know if you have any other concerns.

Thank you.
