Chemo has not helped my mom’s stage 4 lung cancer. Why?

My mom has stage 4 lung cancer with metastasis and a massive pleural effusion on her last chest X-ray. She has been through multiple rounds of chemotherapy, but none of it has helped. The pain is severe and is not responding to narcotics at this point.
