How fast can stage 4 lung cancer spread in a 40-year-old?

Hello doctor,

I hope you are well. I am contacting you because a close friend of mine has recently been diagnosed with stage 4 lung cancer that has metastasised outside of the lungs, she is 40 years old. We want to know more about what this means for the disease’s progression.We would appreciate your help in answering a few questions.

  • What is the normal progression rate of stage 4 lung cancer in a 40 year old?
  • Is younger age more aggressive cancer?

If cancer has spread outside of the lungs it is known as stage 4 lung cancer. Often, stage 4 lung cancer is present before it is even diagnosed. The speed of progression varies. Some tumours progress slowly, while others can progress in just a few months.

Sometimes cancer is more aggressive in younger people, especially if there are certain biological or genetic factors. But age alone doesn’t determine how aggressive the cancer will be. Main factors are the type of cancer, its molecular features and how it responds to treatment.

Younger people often respond better to treatment and this can help. Stage 4 cancer can be slowed down with treatments. Some newer options, such as targeted therapy and immunotherapy (cancer treatments that target specific molecules in or on cancer cells or that help the body’s immune system recognise and attack cancer cells), have helped some patients do better.

The main goals are to keep the cancer under control, to ease symptoms, and to help people live longer. Many patients experience periods of disease stability.
