Hi doctor,

I am a 28 year old male. I have been suffering from hair fall for the last 5 years. I had consulted a leading trichologist chain for treatment and to some extent it did help in controlling the hair fall. But, I have lost a great amount of hair from the front and back. I want to know, what can be done to get back the lost hair? Is hair transplant the only option? I have heard that to effectively perform this, the entire hair has to be shaved off once. Request you to guide me the best way. I have a dry scalp and thin hair. I have uploaded pictures of my hair for your reference. Thank you.

Basically, we can divide your problem in two parts.

  1. Filling of bald area.
  2. How to stop hair fall in existing hair.

So, let us first talk about the bald area that you have got in the front and top of the scalp (attachment removed to protect patient identity).

Yes, hair transplant is the only option to fill it, but I can see that you are still losing hair. So, you can wait for a few years unless you need to take treatment for some emergency occasion. Yes, we need to trim the full hair and it takes at least six months to get results. So, it is a long process. You will be required 3000-3500 graft to fill the front and crown part of the scalp. You have to decide for yourself the suitable time to get a hair transplant. Stopping hair fall is a difficult one. In androgenic alopecia, medicine will show temporary work and nothing can stop your hair fall completely. I suggest you the following treatment. Consult your specialist doctor, discuss with him or her, and take the medicine with consent.

  1. Minoxidil hair oil (5%) in the night over scalp with 10 minute massage.
  2. Follihair one tablet every day.

You can get PRP sitting in clinics (platelet-rich plasma). It is the most advanced procedure non-surgical). It needs multiple sessions with a gap of two weeks between two sessions. Other than this derma roller is another option you can get with PRP. This to the same as PRP needs multiple sessions. You can take Dermaroller and PRP together to increase the effect. But, all these methods cannot stop the hair fall completely. Also, it depends on your scalp nature and how much baldness you will get. It may stop after a few years or it may go till many years to make it completely bald.

What is PRP? Is there any other way apart from shaving entire hair for a transplant? I do not want to shave my hair and wait for six months, as it is difficult with things like job, marriage, etc. I do not wish to know others that I have gotten a hair transplant. Also, currently, I use hair thickening fibers to temporarily hide my bald spots and thinned areas. The issue is that they are expensive and have some drawbacks. Is there any way to hide the thinning areas without waiting so long? Thank you.

PRP is a technique in which we inject blood extract. But, it cannot stop hair fall completely. I think you would like to get a lace hair system. It is very good for people who are not willing to get a hair transplant. Lace is a very popular and very good results. This is a kind of wig but has real hair. It is very comfortable and easy to wear. I personally advise it if a person does not want to get a hair transplant. As you said you are already using fibers to hide baldness, so the Lace hair system will be a very good option for you. You will get it in hair clinics.
