How to treat anxiety and depression post-masturbation?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am 43 male. I started masturbation at the age of 17 and soon developed anxiety, depression, fear, and weakness due to the guilt of severe masturbation. I had taken lots of ayurvedic, homeopathic medicine but no effect. Presently I have severe anxiety, depression, PTSD (post-traumatic stress disorder), and too much sweating with IBS (irritable bowel syndrome) diarrhea problem. I have consulted a psychiatrist and he suggested the below medicine

  1. Paroxetine CR (selective serotonin reuptake inhibitor) 12.5 milligrams - 1-1

  2. Nortimer (Nortriptyline) 25 milligrams - 1-1

  3. Duloxetine (Duloxetine Hydrochloride) 30 milligrams- 1-1

  4. Pronol H (Alprazolam)- 1-1

I am not getting much benefit in IBS diarrhea symptoms and anxiety problems. My doctor suggested replacing Duloxetine with Fluvoxamine (selective serotonin reuptake inhibitor) 50 milligrams for anxiety and IBS problems. Another doctor suggested replacing Duloxetine with Veniz XR 75 mg. In the past, I felt sweating issues with Veniz XR 75 mg. I am confused about which combination of medicines I should choose for my IBS diarrhea, severe anxiety, depression, PTSD, guilt, and fear. I have been suffering from the above problem for the last 20 years. Please help me and suggest me suitable combination of medicine.

Kindly help.

Hello,

Welcome to icliniq.com.

I understand your concern.

You should start taking Fluvoxamine along with Paroxetine (antidepressant) for anxiety and IBS (irritable bowel syndrome). Since you have been suffering from these problems for a long time, it is important to continue the medication for an extended period to see significant improvement.

If you stop the medication within 10 days, you may not experience the desired effects. A treatment duration of at least one month is necessary to observe results. Start Fluvoxamine (selective serotonin reuptake inhibitors) with Paroxetine, along with Propranolol and Nortimer (Nortriptyline).

I hope you are satisfied with my answer. For further queries, you can consult me at icliniq.

Thank you.

Patient's Query

Hello doctor,

Thank you for the reply.

Could you please tell me how Fluvoxamine compares to Duloxetine and Veniz XR, as some doctors have prescribed these medications? Does Fluvoxamine have fewer side effects and work better for anxiety and IBS with diarrhea?

Also, can I continue taking Nortimer 25 mg in the morning and evening along with Fluvoxamine, since it sometimes causes constipation?

Kindly help.

Hello,

Welcome back to icliniq.com.

I understand your concern.

Fluvoxamine is better than Duloxetine and Veniz XR in all aspects. It has fewer side effects and is more effective for anxiety.

Yes, you can continue taking Nortimer twice daily. It is actually beneficial if you experience constipation, as it can help balance your diarrhea.

I hope you are satisfied with my answer. For further queries, you can consult me at icliniq.

Thank you.
