Patient's Query
Hello doctor,
Three years ago, I masturbated for the first time, and semen came out. After that, I stopped masturbation completely. Later, I started having nightfall whenever I ate very spicy food or watched sexual content. Over time, the nightfall episodes began happening every two to three days. Recently, after improving my lifestyle, the number of episodes has reduced to around five to ten. My main concern is that I want to follow celibacy for many months. How can I stop this problem completely?
I feel tiredness and weak eyesight, and pain in my lower back, and due to my nightfall, it has been ongoing for three years, so I feel my focus on studying and learn things is also affected. My age as of 2025 is 19 years old, and I masturbated at the age of 16. I want to know how to know whether my semen is out at the time of urinating or not, which helps me clarify. I am feeling low energy but and my pennis is also shrinking and not getting retention regularly.
My main problem is how to stop my nightfall completely and become celibate and become the person in both terms in physically and mentally. that I was in before doing the wrong thing, masturbation. According to you, how much time is needed to cure completely?
Kindly help.
Hello,
Welcome to icliniq.com.
I understand why you are worried, and I want to reassure you gently but clearly. Nothing you described is dangerous or abnormal for a nineteen-year-old. Nightfall, or nocturnal emission, is a natural body process. It is not caused by weakness, not caused by past masturbation, and it does not damage your eyes, brain, energy, or spine. It only happens because your body continues to produce semen even if you do not masturbate, and when the storage becomes full, the body releases it automatically during sleep. This will continue to happen sometimes, no matter how strict your lifestyle is, because it is a normal biological mechanism. It is not harmful, and it does not reduce testosterone or strength. The tiredness or back discomfort you feel the next day is usually from stress, fear, and disturbed sleep rather than from the nightfall itself.
Your wish to follow celibacy is respected, but celibacy does not mean that nightfall will stop completely. Even monks and spiritual practitioners still experience night emissions. What improves it is reducing stress, improving sleep, staying active, and having a balanced mindset. You are already seeing improvement after changing your lifestyle, which is a very good sign. The remaining episodes will naturally reduce with age and emotional stability. There is no fixed time needed to cure it because it is not a disease, but usually by the mid-twenties, the frequency becomes less on its own.
About your concern that semen may come out during urination, this is rarely the case. Urine and semen use the same passage, but they do not mix. If something comes out during urination, it is usually mucus from the prostate, which is normal. True semen release only happens with orgasm or during nightfall. If you do not feel a wet dream or do not see a sticky white discharge in your underwear, then nothing has happened.
Feeling low energy, lower back pain, or shrinkage of the penis is not because of semen loss. Penile size changes throughout the day based on temperature, anxiety, and blood flow, so temporary shrinkage is normal. Low energy is mostly from stress, overthinking, and poor sleep rather than anything physical.
To reduce nightfall frequency and to feel physically and mentally strong again, focus on the following.
1. Sleep early and wake early, because irregular sleep increases nightfall.
2. Avoid heavy spicy food, long screen time at night, and sexual content.
3. Exercise at least thirty minutes daily, such as running or push-ups.
4. Keep your mind busy in studies, hobbies, or skill-building.
5. Drink enough water and eat a balanced diet.
6. Do not keep checking, worrying, or inspecting your semen. This increases anxiety and makes nightfall more frequent.
You are healthy, and everything you described fits with a normal growing teenage body. With a calm mind and good habits, you will regain your energy and focus. You will not be harmed by past masturbation at age sixteen, and you can still live a celibate life if that is your goal, but nightfall is something you manage, not something you eliminate forever. You are on the right path, and your body will settle naturally over time.
