Why do I not ejaculate after years of masturbation?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 23-year-old boy. I have a habit of masturbation. I have done too much extreme masturbation for seven to eight years on a daily basis. When I have sex with a girl, I do not feel any sensation when I insert my penis in the vagina, and I do not feel the vagina. Also, ejaculation does not happen.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

I am very sorry about your medical condition, and I am glad that you are paying attention to changes in your body.

What you are describing is something many men experience after years of frequent or very intense masturbation, especially if it was done in a specific way, for example, with a strong grip or pressure.

I will tell you what might be happening and what you can do about it safely. The reasons associated with your condition.

  1. One reason is desensitization (loss of sensation). Over the years, repetitive and intense stimulation, especially with a tight grip or friction, can cause your nerves and brain arousal pathways to adapt. This can make normal sexual stimulation, such as vaginal penetration, feel dull or numb.

  2. Your body and brain may get trained to respond only to a certain type of stimulation, such as your hand, position, pressure, or porn use. This can make it difficult to feel arousal or orgasm with a partner.

  3. Heavy porn use can change how your brain processes arousal, which may reduce sensitivity and emotional connection during real sexual activity.

  4. The less common reason is medical or hormonal causes, such as testosterone, nerve damage (for example, due to diabetes or pelvic issues), or certain medications, which can also reduce sensation or affect orgasm.

I advise you on some steps that can be helpful for you. You should follow these steps to manage your condition.

  1. Try stopping for at least four to eight weeks. This helps your brain reset sensitivity and relearn a more natural sexual response. You may experience temporary frustration or mood changes, which are normal during recovery.

  2. If you return to masturbation later, use gentle stimulation without squeezing hard or rushing. Focus on overall body stimulation rather than only the genitals.

  3. Focus on kissing, touch, and emotional connection rather than performance. Do not focus only on reaching orgasm. Try to enjoy the sensation and presence with your partner.

  4. Pelvic floor exercises (Kegel exercises) can help strengthen sexual control and sensation. To do this, tighten the muscles as if stopping urine in the middle of passing it, hold for three to five seconds, relax, and repeat 10 to 15 times. Do these two to three times a day.

  5. Eat a balanced diet, sleep properly, exercise regularly, and reduce alcohol intake and stress. These factors help support hormone balance and nerve function.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
