What causes abdominal pain and heavy bleeding with Filshie clips?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am having abdominal pain with very heavy menstrual bleeding. I had filshie clips implanted a few years back. I can feel one with my fingers.

Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

I am sorry you are going through this. Since you had Filshie clips implanted and are now experiencing severe abdominal pain and heavy periods, a few possibilities should be considered:

Possible causes:

  • Filshie clip migration or complications – if you can feel a clip, it might have moved from its original position, which can cause pain and irritation.

  • Endometriosis or adenomyosis – if your periods have gotten heavier and more painful over time, these conditions could be contributing factors.

  • Uterine fibroids or polyps – these can cause heavy bleeding and pain.

  • Pregnancy complication (ectopic pregnancy) – even with tubal ligation, pregnancy is rare but possible, and ectopic pregnancy is a risk.

  • Pelvic infection (PID) – if accompanied by fever, unusual discharge, or persistent pain, an infection should be ruled out.

What you should do next:

See a doctor or gynecologist as soon as possible – since you are experiencing severe pain and heavy bleeding, they may do:

  • Ultrasound – To check for clip migration, fibroids, or other issues.

  • Pelvic exam – To see if the clip has shifted or is causing irritation.

  • Pregnancy test and blood work – To rule out ectopic pregnancy or other concerns.

Pain management – until you see a doctor:

  • Ibuprofen (if safe for you) – reduces pain and bleeding.

  • Warm compress on the abdomen – helps relieve cramps.

Monitor symptoms – If you have severe pain, dizziness, fever, or excessive bleeding (soaking pads quickly), seek emergency care.

Since you can feel a clip, it is important to get checked soon.

Would you like guidance on managing symptoms while waiting for an appointment?

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
