Can menstrual irregularities cause recurring episodes of nausea, muscle twitching, full-body spasms, and rapid heartbeat?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 23-year-old female with no prior health issues and in great shape. For the past three months, I have been experiencing unusual symptoms. It started in January with feelings of nausea and jitteriness. I noticed constant muscle twitching and would wake up with full-body spasms, accompanied by a sensation of the ground shaking and a fast, pounding heartbeat. These muscle spasms and rapid heartbeat would persist throughout the day, and I continued to feel nauseated. These symptoms lasted for about a week and a half, disappeared, and then returned about a month later. Interestingly, both episodes coincided with the first day of my period and subsided a few days after my period ended. I sought medical help from an endocrinologist and a neurologist, who conducted tests including thyroid and adrenal gland evaluations, a 48-hour heart monitor, and an EMG, all of which came back normal. Basic blood tests also yielded normal results, and I tested negative for Lyme disease. I have a follow-up appointment with the neurologist scheduled for next week. I am desperate for answers and would appreciate any insight into what might be causing these symptoms.

Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

Thank you for your query.

I acknowledge your query and comprehend your concerns.

While it is essential to have complete information about the patient before making a diagnosis, based on the provided details, you may be experiencing premenstrual dysphoric disorder (PMDD). PMDD typically manifests before the onset of menstruation and subsides a few days after the menstrual flow begins. This disorder is characterized by a combination of somatic (physical), psychological, and behavioral symptoms.

I recommend seeking a consultation with a gynecologist or psychiatrist for further evaluation. If you are open to online consultations, you may consider contacting me for further assessment and management.

I hope this answer helped you,

Thank you.
