Hello doctor,

My cousin, who is 15 years old, experienced her first period four months ago, and then again after a month. However, she has not had her period since then. She consulted a gynecologist and underwent an abdominal ultrasound. The ultrasound results showed a small uterus measuring 2.09 × 0.78 inches. She does not have thyroid issues, and her hemoglobin level is 11. Her ovaries appear normal. I am quite concerned. Please help.

I do not see any reason for you to worry. The timing of periods is determined by the hormonal axis between the brain and ovaries. Initially, this axis is not mature, and the trigger for normal ovulation may not occur every month. Therefore, she may experience periods occasionally or not at all. Until the age of 19 to 21 years, there is no need for any evaluation. By this age, the axis gradually matures, and her periods will become regular. Please refrain from becoming overly involved in unnecessary investigations or resorting to hormonal pills.

Focus on the positive aspects: her thyroid hormone secretion is normal, and ultrasound results indicate the presence of a uterus that is still maturing to its adult size.
