I have not had my period in the last four months, and for the past two years, I have been missing my period on alternate months. However, in the past four months, I have not experienced any menstruation. In the past three to four years, I have gained significant weight, going from 119 pounds to 158 pounds.Additionally, I often feel weak and experience gastric problems. I am 26 years old, single, with a height of 5 feet and 2 inches and I weigh 158 pounds. Kindly help.

Based on your medical history, it is likely that you have polycystic ovary syndrome (PCOS). PCOS is a congenital condition influenced by genetics, and weight gain can exacerbate its symptoms. In individuals with excess weight, cholesterol is converted into estrogen hormone, which can disrupt the balance between follicle stimulating hormone (FSH) from the brain and ovarian hormones.
In PCOS, the excess estrogen hormone from fat can confuse the brain and lead to early cessation of follicle growth, resulting in multiple small follicles in the ovaries, known as polycystic ovaries. Common symptoms associated with PCOS include obesity, irregular menstrual cycles, missed periods, heavy periods with clots, spotting, thyroid abnormalities, and prolactin issues indirectly.
Characteristic physical features of PCOS include the development of excess fat around the mid-segment of the body, such as the waist, thighs, and breasts. Other features include excessive hair growth, hair loss, darkening of the skin on the lower half of the face, acne, oily skin, and darkened skin on the back of the neck, inner thighs, and under the surface of the breasts. A family history of diabetes, particularly from the father's side, may also be present.
Weight management is crucial in treating PCOS. The recommended weight range for you can be calculated as follows:
Weight (in pounds) = Height (in feet and inches) - 100
Your target weight should be between 127.868 pounds to 136.687 pounds. Achieving a healthy weight can restore regular menstrual cycles, enhance fertility, and improve pregnancy outcomes.
To reduce weight, it is important to avoid fasting, junk foods, fast foods, and red meat. Instead, focus on consuming low-calorie, high-protein foods. Engaging in regular exercise, including aerobics, yoga, brisk walking, and targeted exercises for the waist and hip area, can be beneficial. Gradual weight loss of 4.41 pounds to 8.82 pounds per month is recommended, as rapid weight loss can be difficult to sustain.
Weight reduction will have a positive impact on hormonal balance and overall health.
To induce regular periods, I suggest you take any one of the following contraceptive pills:
Tablet Novelon (Ethinyl estradiol + Desogestrel).
Tablet Dear 21 (Ethinyl estradiol + Desogestrel).
Tablet Ovral-G (Ethinyl estradiol + Levonorgestrel).
Tablet Duoluton-L (Ethinyl estradiol + Dienogest).
Tablet Intimacy plus 3 (Ethinyl estradiol + Gestodene).
Take one tablet twice a day for 10 days (excluding the 21st pill) to initiate bleeding. From the day of bleeding, continue taking one tablet at night for three to six cycles. This will help regulate hormones and establish regular menstrual cycles. It is important to rule out pregnancy before starting medication by conducting a home pregnancy test.
