How does metabolic surgery help manage diabetes?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

The endocrinologist is suggesting metabolic surgery for my diabetes. I have been diabetic for 12 years, taking four medications plus insulin, and still can not get my numbers under control. My A1C is stuck at 9.2 despite trying everything. I am not very overweight, though I only have a BMI of 32. I thought bariatric surgery was for people who are severely obese. She claims this could potentially reverse my diabetes completely. That sounds like false hope to me.

What exactly happens in my body after this surgery that would help with diabetes? I am terrified of complications. My cousin had a gastric bypass and nearly died from a leak. Is this surgery worth the risk when I am managing okay with medication?

Please advise.

Hello,

Welcome to icliniq.com.

I understand your concern.

Metabolic surgery can be beneficial for diabetes management, even if your BMI (body mass index) is 32. It is not limited to severely obese individuals. The surgery works by improving insulin sensitivity and reducing blood sugar levels through changes in gut hormones and nutrient absorption. Many patients see significant improvements in diabetes control, and some may even achieve remission.

While there are risks, such as leaks and nutritional deficiencies, the benefits often outweigh these, particularly when diabetes is not well-controlled with medications. It is important to discuss these concerns with your healthcare team to make an informed decision.

Please feel free to book a consultation with me if you would like more detailed information on any of these points or if you have additional questions.

I hope this helps you.

Thank you.
