Hello doctor,

I have a canted occlusal plane, a midline shift, and a crossbite.

Could you estimate how long it would take to fix this and whether braces can correct it?

Upon review, these were the findings: you have impaired occlusion, a midline shift, and your mandible is shifted toward the right side. There is a highly placed upper left canine, a posterior crossbite, and a possible prognathic (forward-positioned) mandible. The right lower first premolar is also not in proper occlusion.

Based on this, the provisional diagnosis is Angle’s Class III malocclusion with a protruded mandible, midline shift, a highly placed 23, posterior crossbite, bite plane discrepancy, and 44 not in occlusion. However, there is also a possibility that this could be a skeletal malocclusion rather than purely dental in nature, which is why further evaluation is important.

To properly assess the condition, diagnostic casts and radiographs such as an OPG (Orthopantomogram) and a cephalogram are advised. These help determine whether the problem is primarily dental, skeletal, or a combination of both.

The likely causes could be dental factors, abnormal mandibular growth, or an underlying skeletal discrepancy. If a skeletal issue is confirmed, a surgical evaluation by an orthognathic surgeon may be necessary.

The suggested treatment plan involves premolar extractions after careful evaluation, followed by orthodontic treatment using fixed braces and archwire techniques to align both arches and correct the malocclusion. If the problem is mainly dental, braces alone may be sufficient. If there is a significant skeletal component, combined orthodontic and surgical management might be required.

You would need to follow up regularly with an orthodontist, and the estimated duration for correction with braces is approximately two years. Overall, the prognosis is considered good to fair, depending on the extent of skeletal involvement and how well the teeth respond to treatment.
