I am 35, female. Why do I get frequent periods before periods?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am 35 and get migraine attacks around four to five times a month, mostly before my periods. My recent MRI brain was normal, and my vitamin D is 19 ng/mL. I am using Sumatriptan during attacks, but still need to lie down for hours. Would starting a preventive medication like Propranolol or Topiramate help reduce frequency? Are newer CGRP injections better for hormonal migraines specifically? Can low vitamin D or poor sleep patterns make these attacks worse? My BP is normal, but I sometimes feel dizzy after using Triptan. Should I take it with food or lower the dose slightly?

Thanks.

Hi,

Welcome to icliniq.com.

I can understand your concern.

I am deeply concerned about your worries. You should avoid common food triggers:

  1. Alcohol: Especially red wine and beer.
  2. Caffeine: Found in coffee, tea, and colas.
  3. Aged and fermented foods: Aged cheeses (like Parmesan, blue cheese), fermented soy products (like soy sauce), and pickled foods (like sauerkraut, olives).
  4. Processed and cured meats: Often contain nitrates and nitrites, such as pepperoni, hot dogs, and bacon.
  5. Chocolate: Contains compounds like tyramine and caffeine.
  6. Food additives: Monosodium glutamate (MSG) and artificial sweeteners like aspartame.
  7. Certain fruits: Citrus fruits, bananas, raspberries, dried fruits, and avocados.
  8. Nuts and nut butters: Can be a trigger for some individuals.
  9. Other foods: Tomatoes, onions, and some beans are also frequently reported as triggers.

How to identify your triggers:

  1. Keep a food diary: Track what you eat and when your migraines occur to identify patterns.
  2. Try an elimination diet: Remove suspected foods for a month to see if your migraines improve, then reintroduce them to confirm the trigger.

Management Plan:

  1. Start taking the tablet Migley 20 mg (Propranolol) only when you have a severe headache.
  2. Take a tablet of Naproxen 500 mg for 10 days.
  3. Take a tablet of Omeprazole 20 mg half an hour before breakfast.
  4. Take a tablet of Cholecalciferol 200 mg once every 15 days for one year.

Yes, OCPS (oral contraceptive pills) they cause migraines after taking a long period, or they can exaggerate the symptoms and recurrence of the migraines. Use the triptens after taking the food so that you do not feel dizzy.

I hope this information will help you.

Thanks.
