Why does miscarriage occur after early heartbeat detection?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I need help from a gynecologist. My wife had two miscarriages. During the second miscarriage, a heartbeat was detected at six weeks, according to the doctor, but after 15 days, another checkup showed no heartbeat. The machine still indicated a six-week pregnancy, but in reality, it was eight weeks.

  1. What should we do?

  2. Would it be helpful for my husband to undergo a semen analysis test?

Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

I am sorry to hear about the miscarriages your wife has experienced. Recurrent miscarriages can be difficult, and it is good that you are seeking advice.

There are several possible causes for recurrent miscarriages, and both partners should undergo evaluation to identify any underlying issues. A semen analysis can be part of the evaluation, as male factors such as sperm quality and DNA (deoxyribonucleic acid) fragmentation can sometimes contribute to miscarriage.

In addition to semen analysis, here are a few other areas that should be explored:

  1. Chromosomal analysis: Both partners can undergo genetic testing to check for chromosomal abnormalities that might lead to recurrent miscarriage.

  2. Hormonal and endocrine factors: Your wife's hormonal levels, thyroid function, and insulin resistance should be assessed. Conditions like PCOS (polycystic ovarian syndrome) or thyroid imbalances can sometimes cause miscarriage.

  3. Uterine abnormalities: A detailed ultrasound or hysteroscopy can check for structural abnormalities in the uterus, such as fibroids or a septum, that could affect pregnancy.

  4. Blood clotting disorders: Tests for clotting disorders like antiphospholipid syndrome should be done, as they can lead to miscarriage by affecting blood flow to the placenta.

  5. Lifestyle factors: Both partners should look at factors such as diet, smoking, alcohol consumption, and stress levels, as these can also impact pregnancy outcomes.

The good news is that after finding the root cause, many couples go on to have successful pregnancies. Your husband's undergoing a semen analysis could be helpful as part of this comprehensive assessment.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
