Hi doctor,

I am a 34-year-old breastfeeding mother to a 6-week-old baby. I had little nasal congestion during pregnancy, which got aggravated due to two to three episodes of acid reflux while sleeping at night when I was in the third trimester. This led to severe nasal congestion and post-nasal drip. A nasal endoscopy was done, which showed rhinitis features that could be allergic or non-allergic or from pregnancy hormones. I am eating Montair FX and using isotonic saline spray to alleviate the symptoms. What can I do to find quick relief?

What is the reason the ENT doctor mentioned for nasal congestion? What medications did the doctor suggest after doing a nasal endoscopy? Continue the recommended medication. Thank you for the reply.

Slight nasal congestion was there from the second trimester, but it increased in the third trimester. Post-nasal drip started only after having acid reflux. Nasal congestion is not there anymore; I now have post-nasal drip. Due to this, I either have to swallow the mucus or clear my throat several times a day. The ENT doctor told me it could be due to allergies or pregnancy rhinitis as there was inflammation in the nasal passages. The doctor put me earlier on Montair LC (Levocetirizine and Montelukast) and now on Montair FX (Montelukast and Fexofenadine). I was also given corticosteroid nasal sprays, but they were doing more harm than good. Also, the doctor mentioned the inflammation could also be due to the overuse of Otrivin (Xylometazoline hydrochloride and Ipratropium bromide) nasal sprays. So, the only issue I have now is post-nasal drip.

You have symptoms of post-nasal drip. Causes of post-nasal drip include:

  1. Allergy.

  2. Sinus infections.

  3. Pregnancy.

  4. Certain medications.

  5. Acid reflux.

You have already done a nasal endoscopy, one of the crucial investigations to look for nasal pathology. You are on antiallergic medication also. Stop the Otrivin (Xylometazoline hydrochloride and Ipratropium bromide) nasal drop I prescribed above. Since you are not getting relief with Montair FX (Montelukast and Fexofenadine), I suggest taking antacid medication also. Take a tablet of Pantocid (Pantoprazole) 40 mg once a day on an empty stomach for a week and watch for the improvement in symptoms. Avoid spicy foods and drinks for at least three hours before bedtime, keep your head elevated six to eight inches above your body at bedtime, etc., to prevent acid reflux.

Thank you for the reply doctor,

My post-nasal drip is not going away. I have taken many anti-allergic medications during my pregnancy, including levocetirizine and Montelukast, but I still did not get any relief. My gynecologist also prescribed Pantoprazole twice a day and Gaviscon before bedtime, but I still experienced acidity during my pregnancy.

Although the acid reflux subsided after delivery, I had acute acidity up until the last day, which was 35 days ago. Currently, Montelukast along with Fexofenadine is also not providing any relief.

You have tried medications that are generally prescribed for post-nasal drip. You have also had a nasal endoscopy, which came back normal. What exact symptoms are you experiencing now? I would suggest continuing Pantoprazole once a day for another week. Do not take Montelukast along with Fexofenadine now. There is no need to do lukewarm gargles.

My current symptoms are post-nasal drip, which causes me to clear my throat many times throughout the day, and light nasal congestion. My nose is free from congestion when I wake up in the morning, but one nostril becomes slightly congested later, and then the nasal dripping starts. My nasal endoscopy report showed mild DNS (deviated nasal septum) to the right with inflammation in the nasal passages.

So far, you have tried antiallergic and antigas medications to treat your current symptoms, and they should have improved by now. I am aware of your nasal endoscopy report, and mild DNS (deviated nasal septum) generally does not cause such chronic nasal congestion symptoms. Now, the question is what you should do next. Based on your history, I suspect the cause of your post-nasal drip is nasal congestion (not acid reflux, as you did not mention any symptoms of acid reflux). You noted that the nasal drip starts after one of your nostrils becomes slightly congested. Congestion usually occurs due to infection or allergy. You should avoid exposure to dust and stay in a clean area with fresh ventilation.
