Hello doctor,

My nasal passage was cauterized four months back. Two days later I began having lots of yellow pus like discharge from my nose. It mostly was being expelled via my throat with sputum. The cauterized site was yellow and there was yellow pus like all over. I have been prescribed Amoxyclav 625 for seven days, Cefuroxime 500 for seven days. The cauterized site was still not healed and was still having yellow discharge. I then consulted an ENT doctor who took a look at it and gave me Linezolid 600 for 10 days. The infection of the cauterized site is still not healed and still oozing with yellow pus-like discharge although there is slight improvement. I have low grade fever on and off. I feel I may be having an infection in my nose that is resistant to antibiotics. Can you please help me and advice me on how to treat my nasal passage? And also what other investigations should I do? I am worried about the infection whether it could harm me as it continues.

The cauterized site you have mentioned might be infected as you said.

1. The best treatment now in this situation is take a sterile swab and touch the swab with the pus and send it for culture and sensitivity, it will definitely give the medication sensitive for your bacteria.

2. Meanwhile start injectables preferably Ceftazidime 1000 mg and injection Amikacin 500 mg after test doses for five days.

3. Start application of Neosporin ointment on the site.

What other options are there to treat my infection as it has been quite long that I am having this infection in the nose. I was prescribed T- bact ointment to apply and I am applying it since eight days.

I think the swab culture and sensitivity are the best options as you might be resistant to other organisms.

1. I advise you to start injectables as they remarkably will reduce the infection.

2. The injectables I mentioned cover both gram positive and gram negative organisms, this itself may cure your disease.

3. One more suggestion is not to prick or self clean the nose unless the pus is trickling outside. It is not a rare condition and can come to people very frequently.

I have seen the attached files, you are suffering from infection and I think you should start the antibiotics as I have suggested (attachment removed to protect patient identity). It will subside with the antibiotics and also the T-bact ointment. It is the superficial infection of that specific site and it is not a very serious condition and can be cured with those antibiotics itself and after that you can use the T-bact ointment.

Do not worry regarding the infection, we have treatment even for MRSA and VRSA and I am sure your infection is not one of them and I can see the infection reduced compared to yesterday. Injection Ceftazidime 1000 mg and injection Amikacin 500 mg twice daily for five days intravenous injection by a doctor or a person who can give you the dose after giving proper test doses and you are proved not allergic to the drug .
