Hello doctor,
My girlfriend is a 19-year-old and has nipple pain. She says it is very sensitive to the point it hurts nowadays. She has irregular menses that requires taking some birth control pill-like medication to force her menses to come every three months. Is it a sign of pregnancy or other health issues due to the sensitivity of her nipple that causes pain? Her breast has also grown a cup size bigger too. Kindly suggest.

Due to abnormal cycles or hormonal imbalances, breast tenderness can happen. Overdue and unscheduled irregular periods can cause breast tenderness, mood variations, etc. If she has gained weight and has abnormal hair growth along with irregular cycles, this could be a sign of polycystic ovaries and need investigation, precautions, and treatment accordingly. She should have regular oral contraceptive pills for three months to regulate her periods. It will act as a contraceptive to prevent conception and help the reproductive system and breasts develop normally. For now, have a beta hCG (human chorionic gonadotropin) blood pregnancy test to check pregnancy status. If it comes negative, have menstrual induction with the tablet Primolute N (Norethisterone) 10 mg twice daily for five days and then wait one to two weeks for bleeding. Consult your specialist doctor, discuss with them, and take medications with their consent. On the second day of the bleeding, have blood tests for serum progesterone, testosterone, insulin, thyroid function test, FSH (follicle stimulating hormone), and LH (luteinizing hormone). A transvaginal scan is to be done after periods of days. Collect all reports and discuss them here. Walk daily and do exercise and maintain normal BMI and weight. Have a healthy diet with less salt, sugar, oil, and spices.
