Hello doctor,
My 25-year-old brother got sick after a family barbecue last weekend. We thought it was food poisoning at first, but now it has been six days, and he is still throwing up and has nonstop diarrhea. He cannot keep any fluids down and has lost so much weight. Is this norovirus? Does he need specific medications for it, or does it get better on its own? Should he be taken to the ER for fluids or just let it pass? He now looks completely drained. How long does this usually last?

I understand your concern.
As per your symptoms and duration of disease and on set, it might be acute gastroenteritis, but in his case, he is allergic to a high protein diet, which probably led to stomach upset. I am prescribing some advice and medication and kindly follow up for three to five days. The probable causes are infective or electrolyte imbalance.
Investigations to be done:
CBC (complete blood count).
LFT (liver function test).
Serum electrolyte.
Stool R/M test and stool C/S test.
Probable diagnosis: Acute gastroenteritis
Treatment plan:
Tablet Ciplox 500 milligrams (Ciprofloxacin) twice a day for five days.
Tablet Metronidazole 400 milligrams three times a day with meals for five days.
Tablet Pan D (Pantoprazole) before breakfast for ten days.
ORS (oral rehydration solution).
Tablet Loperamide 2 milligrams twice a day for three days.
Capsule Bifilac (Bacillus mesentericus, Clostridium butyricum, Lactic acid bacillus, and Streptococcus faecalis) twice a day for five days.
All these medications need to be taken after consulting a medical specialist.
Regarding follow-up:Follow up after five days. If there are signs of stress, it might require admission for IV (intravenous) fluid and further investigations.
Preventive measures:A soft diet should be consumed. Drink plenty of fluid. Avoid fast food, oily, and spicy food.
