Hello doctor,
I got a norovirus infection, and diarrhea has not stopped for two weeks. I had a kidney transplant, so my immunity is low. My nephrologist said the virus can stay long in people like me.
Is that true?
What medications or precautions can I take?
Can this damage my kidneys or graft?

Norovirus infection in kidney transplant patients under immunosuppressants can be more dangerous than for the general population. The immunosuppression weakens the body's ability to fight off the infection, potentially leading to more severe symptoms and complications.
Specifically, in this population, norovirus can cause prolonged, chronic diarrhea, dehydration, and, in some cases, acute kidney injury (AKI). Therefore, it takes a longer time to resolve these types of patients, sometimes months. Recently, there have been no definitive antiviral treatments or vaccines, but Nitazoxanide and immunoglobulin have shown efficacy in limited cases.
For diarrhea:
You need to take oral rehydration solution (ORS) and electrolyte drinks.
Check for signs of dehydration: dizziness, decreased micturition, and dry mouth. If any of these are present, immediately seek medical attention.
If you keep everything down or cannot take fluids orally, you need to be admitted to the hospital for intravenous fluids.
There are also chances of both acute and chronic graft rejection. Sometimes you need to lower immunosuppressants; in that case, the chance of rejection also increases.
The only precautions you can take are
Cleaning surfaces thoroughly.
