Hello doctor,

I am a 32-year-old woman, ten months postpartum, and I am currently struggling with obesity. Additionally, my doctor has mentioned that I have a bulky uterus, and I have been experiencing pain on the left side of my C-section site, which has been concerning me.

I am worried about how my weight might be affecting my recovery and overall health, especially since the pain seems persistent. Could the pain be related to my C-section, or is it potentially linked to my obesity or uterine condition?

What steps can I take to manage my weight and improve my health while balancing the demands of caring for my baby?

Also, are there specific treatments or investigations I should consider for the pain and bulky uterus? My test results also show debris in the bladder and bilateral ovarian cysts of 1.06 X 0.90 inches in size on the right side with a volume of 0.45 ounces and 0.38 ounces on the left side. Kindly help.

The pain on the left side of your C-section site could be due to scar tissue (adhesions) that might form after surgery.

These adhesions can sometimes cause discomfort or pain, particularly during physical activity.

The pain could also be related to your bulky uterus, which may be pressing on certain structures, leading to discomfort near the surgical site.

Obesity (excessive body weight) can also exacerbate pain because it puts extra pressure on your abdominal and pelvic area. Obesity could contribute to delayed healing and increased strain on the tissues surrounding your C-section site.

As for your bulky uterus, it can be a sign of

  1. Fibroids (muscular tumors that grow in the wall of the uterus).

  2. Adenomyosis (a benign uterine disorder in which endometrial glands and stroma are pathologically demonstrated in the myometrium).

Both of which can cause discomfort and contribute to the overall feeling of bloating or heaviness in the pelvic area. The ovarian cysts you mentioned also need to be monitored, especially since they are of considerable size. Cysts of this size are not uncommon but can cause pain if they become large or rupture.

A thorough follow-up with your gynecologist is important. Regarding the debris in the bladder, this could indicate an infection.

A urinary tract infection (UTI) or post-surgical complications could sometimes lead to this finding. Get a urinalysis done.

To manage your weight, I suggest the following

  4. Follow up with your gynecologist.

  5. A physical examination by your doctor is a must.

  6. A pelvic ultrasound (a noninvasive diagnostic exam that produces images that are used to assess organs and structures within the female pelvis) or even an MRI (magnetic resonance imaging) (a type of diagnostic test that can create detailed images of nearly every structure and organ inside the body) to evaluate the C-section site, the uterus, and the ovarian cysts more thoroughly.

  7. Hormonal evaluations are done to determine the level of hormones.
