I have OSA. Will I need CPAP even after losing weight?

Hello doctor,

I am 44, overweight, and recently diagnosed with obstructive sleep apnea after a sleep study showed an AHI of 32, indicating severe OSA. I snore loudly and often wake up tired despite sleeping seven hours.

My doctor advised CPAP therapy, but I am finding it very uncomfortable to use. So my concerns are -

  1. Are there alternative treatments like oral appliances or positional therapy that work effectively in severe cases?

  2. Also, if I lose weight significantly, can OSA be fully resolved, or will I still need CPAP lifelong?

  3. Should I be concerned about long-term heart or blood pressure problems if I delay treatment much longer?

So, with an AHI (apnea-hypopnea index) of 32, your obstructive sleep apnea is in the severe range. CPAP (continuous positive airway pressure) remains the most effective treatment to reduce apneas, improve sleep quality, and lower the risks of complications.

Alternatively, oral appliances (mandibular advancement devices) or positional therapy may help in mild to moderate cases. Still, they are usually less effective in severe obstructive sleep apnea (OSA), so they might not fully control your condition on their own.

Significant weight loss can significantly improve OSA and, in some people, even lead to resolution. However, this varies depending on structural factors in the airway that can still cause obstruction, so CPAP may still be needed even after weight reduction.

Untreated severe OSA is linked to serious long-term risks, including high blood pressure, atrial fibrillation, stroke, and heart disease, so delaying effective treatment is not advisable.

If CPAP is very uncomfortable, it is worth discussing mask adjustments, humidification, or pressure modifications with your sleep specialist before moving away from it, as optimizing comfort often makes it more tolerable and ensures you are adequately protected from the cardiovascular risks of OSA.
