Hello doctor,
I cannot get an orgasm from a partner, and the thing is, it is 100 % impossible for me to get an orgasm from internal stimulation. All the partners I have been with are extremely good at what they do, and it feels amazing, but I can never get to the point of orgasm. I am capable of it with myself when I do not have to worry about my facial expressions, close my eyes, and not having to worry about being sexy, and making sex noises. I want to cum for my partner so bad, we have been together for almost a year, and I fake my orgasm every time. He thinks he makes me cum every time, and it makes me feel really bad lying to him about that. I want to actually orgasm for him.

I can see you are always under pressure while having sexual intercourse. If you are not completely relaxed and your mind is thinking about other things (how your facial expressions are or if you are looking sexy, etc), then you will not be able to focus on sex, and you will not achieve orgasm. When you do not achieve orgasm, it further creates new stress, and you get trapped in a vicious cycle of expectations and disappointments. I would suggest you start relaxation exercises like deep breathing exercises. If this does not solve the problem, meet a psychologist or psychotherapist for a detailed evaluation of the problem and further treatment.
