Hello doctor,

I have an overactive bladder and take Mirabegron daily, but I want to get pregnant now. My urologist did not know if it was safe during pregnancy.

  • Can an overactive bladder worsen in pregnancy or affect labor, too?

  • If I stop medicines, I am scared I will be constantly uncomfortable or leaking.

  • Is there something safer I can use during this period?

Currently, Mirabegron is not recommended during pregnancy due to limited safety data in humans. Studies have shown some adverse effects at high doses. Because we do not yet have robust human studies confirming its safety, most clinicians advise discontinuing it when planning conception or during pregnancy.

• Overactive bladder can worsen during pregnancy due to hormonal changes, increased pressure on the bladder from the growing uterus, and fluid shifts. Not all women experience worsening, and some adjust with conservative strategies.

• Overactive bladder itself does not typically affect labor directly, though frequent urination and urgency can cause discomfort.

Since most overactive bladder medicines are not well studied in pregnancy, we usually prioritize non-pharmacological methods, such as:

• Timed voiding (going to the bathroom on a set schedule).

• Pelvic floor muscle training (Kegels), ideally under the guidance of a pelvic physiotherapist.

• Bladder training techniques.

• Lifestyle modifications, such as fluid spacing and reducing caffeine or acidic foods.

If symptoms become very severe, a specialist urogynecologist or maternal-fetal medicine team can consider options tailored to your stage of pregnancy.
