How can panic attacks due to glossophobia be managed?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

When I was in eighth grade, my teacher asked me to begin reading on a topic, and from that time, it started. I suffer from glossophobia, which is just a fear of public speaking. And that is on a very high level. I am having panic attacks, my heart rate is racing, I am shaky in my hands, legs, and lips, and my voice has suddenly become unrecognizable and slow as if I cannot breathe. I have tried Inderal, Rivotril, and Citanew. I am also on insulin for diabetes.

Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

It sounds like you are dealing with severe anxiety or panic attacks, particularly in public speaking or situations where you are required to speak in front of others. Glossophobia (fear of public speaking) can trigger a variety of physical and emotional reactions, and it seems like your symptoms have escalated over time, which must be challenging.

The symptoms you are experiencing—rapid heart rate, shaking, breathlessness, and changes in your voice are typical of panic attacks or severe anxiety. These physical manifestations can be terrifying, especially when they interfere with day-to-day activities, like speaking in front of others.

Here are a few steps that may help you manage this situation:

Speak to a healthcare professional: Given the intensity of your symptoms and their persistence for several years, consulting with a doctor or therapist is crucial. You may be dealing with a combination of anxiety and other underlying issues, and a professional can guide you toward treatment options.

Cognitive behavioral therapy (CBT): CBT is a very effective treatment for anxiety and panic attacks. It helps you address the thought patterns that contribute to your fear and provides strategies to manage your physical reactions.

Breathing exercises: Practicing deep breathing techniques can help regulate your breathing and calm your nervous system when you feel anxious. Focused, slow breaths can lower your heart rate and reduce the feeling of panic.

Medication adjustment: While you have tried medications like Inderal (Propranolol) and Rivotril (Clonazepam), it may be worth revisiting these options with your doctor to see if your current regimen is the best for your needs. There are other medications specifically designed for anxiety and panic disorders, and your doctor may suggest different combinations or adjustments.

Gradual exposure: You might consider gradually exposing yourself to public speaking situations in a controlled, less stressful environment. Start small, like speaking in front of a mirror or with a friend, and gradually build up to larger audiences. This can help desensitize you to the fear and give you more control over your reactions.

Mindfulness and relaxation: Meditation, yoga, and other relaxation techniques can help you stay grounded and reduce anxiety over time.

It is important to remember that you are not alone in this, and with the right support, many people manage to overcome or significantly reduce their anxiety. Do not hesitate to reach out to a healthcare provider who can give you a personalized treatment plan.

I hope this helps.

Thank you and take care.

Regards.
