Hello doctor,
I am a 55-year-old male, and I had papillary carcinoma. I had thyroid cancer with lateral neck node involvement two years ago and had a total thyroidectomy. In the last year, my Tg level has increased slightly. Also, growth has shown up in the last year and has improved somewhat. Is there anything to be concerned with? Would it not be possible for my thyroid cancer to go back if the Tg level has gone up? Kindly help.

I am sorry about your medical condition. Regarding your symptoms, I believe your thyroid cancer has resurfaced, and this is called a relapse. Since the latest mass growth is around vital structures, making it challenging to biopsy or expose for surgical removal, there will be no need for such a procedure. I would instead suggest you undergo chemotherapy. The papillary cancer is the worst among other thyroid malignancies. Please see your endocrinologist for a possible start of chemotherapy.
