Hi doctor,

I am a 37-year-old female who recently had very fast heart rate episodes. This has been happening on and off for the past four weeks. It comes on very suddenly when it happens, and my heart rate goes up to 165 bpm. It is very frightening. I have been to my doctor twice, but he thinks it is anxiety, and he gave me tablet Propranolol 80 mg modified release capsules to take every day. My heart rate is fine when I take them, and I feel calm. However, I am scared if it is not anxiety, and there is something very seriously wrong with my heart. I had two echocardiograms last year, and they were normal. The cardiologist said there was nothing wrong with my heart and not to worry. I am a very anxious person, and I have been worried lately. It has been getting worse, severe to be exact. I do not know if this fast heart rate is anxiety or if something is seriously wrong with me? Would the echos last year pick up if something wrong caused rapid heart rate? I was going out of my mind with worry and scared to live my life. Please help.

I understand your concern. Sudden episodes of fast heart rates and sudden cessation with normal echocardiography are most commonly supraventricular tachycardia (SVT). To confirm this rhythm, you have to have an ECG done when you have palpitations or fast heart rates. Alternatively, you can have Holter monitoring in which ECG (electrocardiogram) will be continuously recorded for 24 hours or more, recording such rapid heart rate episodes. This is not a life-threatening or dangerous thing. I suggest you continue tablet Propranolol. Despite this medicine, if you still have paroxysms of SVT, I suggest electrophysiologic study and catheter ablation procedure be done to cure it permanently. First, you have to confirm whether it is SVT (most probably) or another problem. I hope this helps.

I understand your concern. Anxiety can be a reason, but we will label anxiety after excluding possible organic causes. At this age, 20 to 40 years, SVT is common. Sudden onset and sudden offset of fast heartbeats can be SVT. This usually occurs in paroxysms and occurs for a few minutes so getting ECG done is difficult. Sometimes, the episode lasts longer, so ECG is recorded, and cardiac rhythm is captured and diagnosed. SVT is not dangerous. I suggest you get an ECG done, especially when you have such an episode. Do you have a thyroid issue, diabetes, hypertension, or kidney issue? I hope this helps you. Kindly follow up with the answer.

Thanks for your reply. No, I do not have any mentioned problems, but I have multiple sclerosis. I was diagnosed five years back. My heart palpitations occur for about 60 minutes and get reduced after taking tablet Propranolol. But my doctor says it is due to anxiety. Please help.

I understand your concern. Anxiety rarely causes the heart rate to rise to 165 beats per minute. However, a panic attack, that is an extreme attack of sudden anxiety, may cause a high heart rate. Your fast heart rate episodes last for around an hour, so doing an ECG is possible. When you have such an episode, get an ECG done. This will tell whether it is SVT, sinus tachycardia, atrial fibrillation, or others. I hope this helps.

Thank you for your reply. The doctor felt my pulse when I had a fast heart rate and said it was very fast but regular and listened to my heart, and there were no echos, and it sounded normal. Please help.

I understand your concern. Atrial fibrillation has an irregular pulse, and SVT (supraventricular tachycardia) and sinus tachycardia have a regular pulse. ECG (electrocardiogram) is indicated in any fast heart rate cases. Echocardiography is also normal, so it is not severe in your case. Do not take stress, and be relaxed.
