Hello doctor,
I have two kids and recently diagnosed with PCOS/PMOS. My last ultrasound showed I had 30 follicles in my left ovary and 25 in my right, with only one mature follicle, we have been trying medicated times intercourse with a trigger shot and it is not working. I am not getting pregnant. Why does PCOS/PMOS affect with fertility so bad? Took Letrozole 2.5 mg three tablets once a day from day 3 to 7 and then did a trigger shot a week back- Ovidrel.

PCOD/PMOS is a polycystic ovarian disease/polyendocrine metabolic ovarian syndrome which causes increase testosterone levels in women with androgenic features and multiple small cysts in ovaries. It is one of the most common reasons for irregular menses, especially oligomenorrhoea and primary or secondary female infertility. The increased androgen feels do not allow the follicles to mature and hence ovulate and also if delayed ovulation occurs, the quality of the egg is not optimum to fertilize and lead to pregnancy. You shall need to be placed on low dose oral contraceptive pills with Myoinositol supplementation with a reduction in weight with exercise and diet modification (majorly) and increasing your daily water intake. All these for 3 to 6 months have been advocated for treatment of PCOD/PMOS.
