Hello doctor,

I am 18 years old and had trimmed my pubic hair recently. I am getting big pimples on my ball sacks which started about a week ago, at first my balls were only itchy but now they have become large pimples. Kindly suggest.

I have read your query and can understand your concern. I have checked the picture (attachment removed to protect the patient's identity). I would like to know if there was any unprotected sex - one needs to think of sexually transmitted infection. I suggest the following tests:

1. Treponema pallidum hemagglutination (TPHA) test.

2. HPV (human papillomavirus) DNA (deoxyribonucleic acid) PCR (polymerase chain reaction) test.

3. Herpes simplex virus (HSV1 and HSV2) serum IgG (immunoglobulin G) and IgM (immunoglobulin M) test.

4. Chlamydia trachomatis IgG test.

5. HIV (human immunodeficiency virus) one and two antibody screening tests.

6. HbsAg (hepatitis B surface antigen) test and complete urinalysis.

If not, it can be a simple bacterial infection for which you can use Mupirocin ointment for local application and a tablet of Doxycycline and Ascorbic acid 100 mg twice a day for five days. For itching - you need to think of fungal infection - Beclomethasone and Clotrimazole ointment for local application and a tablet of Cetirizine hydrochloride once a night. Make sure you wash your genitalia twice a day, morning and night, and use normal water, no cold or hot water. Avoid using heavy pH (potential of Hydrogen) soap.
