Why are my peptic ulcers not healing despite treatment?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 21-year-old woman. I have been suffering from chronic gastritis, peptic ulcers, and vomiting blood for around three years. I have made dietary and lifestyle changes as suggested by my doctor. However, the ulcers have not yet healed despite taking medications.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

If ulcers and chronic gastritis have not healed after three years despite proper treatment, it usually means there is an underlying cause that has not been fully addressed. At this stage, instead of continuing the same medicines or trying home remedies, it is very important to reassess the condition with a gastroenterologist.

Non-healing ulcers are mostly due to Helicobacter pylori (H. pylori) infection. In some cases, the infection may not be completely eradicated, or the bacteria may develop resistance to commonly used antibiotics. That is why you have to repeat the test (breath, stool, or biopsy) to confirm whether the infection is still present.

If H. pylori is detected again, you need to take an anti-H. pylori regimen for 14 days, widely available in the market as the Pantocid HP kit (Amoxicillin, Pantoprazole, and Clarithromycin). It is very important to take the medication exactly as prescribed, without missing doses, to ensure complete clearance of the infection. Discuss with your doctor to start on it.

Also, I would advise you to get an anti-TTG (tissue transglutaminase) antibody test done, which is a blood test to rule out celiac disease (an autoimmune condition where the immune system reacts to gluten), which can sometimes contribute to persistent gastrointestinal symptoms and poor healing.

If you experience fresh bleeding (vomiting blood or black stools), dizziness, or weakness, you should seek immediate medical attention.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.

Patient's Query

Hello doctor,

Thank you for your reply.

I have had the TTG antibody test done, and the result is negative. I have used the HP kit for 14 days. However, there are no visible results, and I am still suffering from bloating, gas, indigestion, and weight loss.

Please help.

Thank you.

Hello,

Welcome back to icliniq.com.

I have gone through your query and understand your concern.

In that case, my diagnosis would be irritable bowel syndrome (a condition that affects the stomach and intestines, also called the gastrointestinal tract).

I would advise the following:

  1. A proton pump inhibitor, such as Pantoprazole or Rabeprazole, can help reduce stomach acid.

  2. Avoid fatty and spicy foods, and prefer a soft, bland diet.

  3. You can take a smooth muscle relaxant like Mebeverine for stomach pain, if present.

  4. In refractory cases, medications like Chlordiazepoxide and Clidinium bromide may be used if the condition does not respond.

This is my best opinion based on the available details.

I would also advise you to consult a doctor in your city and share this opinion with them.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
