What is the effective treatment for a perianal abscess that is not responding to treatment?

Hello doctor,

I am a 25-year-old male who has been suffering from an abscess in the perineum for the past seven months. I have not consulted any doctor as I only found a hole outside, but liquid pus and blood come out of it regularly. Recently, that abscess became big and started to drain, so I went to the hospital. They conducted a few tests and prescribed an antibiotic tablet of a single type for seven days. After using the tablets, the big abscess disappeared, but it formed again as a hole, releasing pus and blood. The doctors at the hospital told me to come back after finishing the course of tablets. Later, I consulted another doctor and was advised to use Ciprofloxacin 500 mg for seven days and Mupirocin for 14 days. I used Ciprofloxacin for seven days, but I did not get relief from the abscess. Can you please guide me further and prescribe any medications?

Perianal abscesses, especially small ones, can be managed with good antibiotics such as Augmentin (Amoxicillin and Clavulanic acid) 625 mg twice a day and sitz baths (by adding Betadine to water in a tub and sitting in it for five minutes twice a day). However, most of the time, surgical intervention is required.

I suggest you start Augmentin, but if the condition persists after that, it is advisable to visit a surgeon in the clinic.

Do I use povidone 10 percent instead of Betadine? I heard both are the same.

Povidone iodine and Betadine are the same. You can use a 7 to 10-percent strength solution. Put two caps of it in a tub of water and sit in it for five minutes.

Previously, I used Augmentin 875/125 mg for a week, and after a gap of two weeks, I started Ciprofloxacin 500 mg. Should I use Augmentin 625 mg again, or have the bacteria become resistant to it?
