Hello doctor,

I was recently at the hospital for chest pain and took an ECG twice and other tests (including a CT scan, blood tests, D-dimer test, heart rate, and blood oxygen), but they could not find anything wrong. Yet it hurts to breathe and feels like my heart is pounding, causing my upper back to hurt. It feels as if my chest is being squeezed, and sometimes I have a burning feeling. So, they suggested Ibuprofen and Pepcid for the pain. I am currently on the inhaler, birth control pills, vitamin D tablets, and Singulair. Please help.

I understand your concern and would like to explain that your clinical symptomatology seems more likely to be related to pleuro-pericardial inflammation. Acute pericarditis is highly suspected. Any recent viral or other systemic infection could be a potential cause. Besides the above-mentioned medical tests, a cardiac ultrasound would help investigate pericardial inflammation or effusion. A troponin test is also required. If evidence of pericardial inflammation is sustained, cardiac magnetic resonance would be the recommended test to confirm the diagnosis. It would help if you discussed the issues mentioned above with your attending physician.

I did get a troponin level check. It was 0.10, which I was told was good.
