I understand your concern.

I reviewed your case history and the attachments (attachments removed to protect the patient's identity). There is slight inflammation of the gums over the right lower last molar. This might be due to food impaction and inability to properly brush in that area or to impingement of the right upper last molar. There are minor cavities in your lower first two molars that need filling. You should get an X-ray done, properly brush that area (in a circular motion by placing the brush over the tooth), and use povidone iodine mouthwash. I would like you to answer some of the questions:

  1. Do you have any flu-like symptoms?
  2. Does it hurt while clenching your tooth?
  3. Do you have difficulty swallowing?
  4. Does it bleed while brushing, or is there any pus discharge in that area?

Kindly revert with the X-ray and answers to the above questions for further diagnosis and treatment plan.

The Probable causes

1. Food impaction. 2. Unable to brush and maintain that area properly.

Treatment plan

1. Capsule Amoxicillin 500 mg for three days. 2. Tablet Metronidazole 400 mg for three days. 3. Tablet Zerodol SP (Paracetamol and Aceclofenac) for three days. 4. Tablet Pantoprazole 40 mg for three days. 5. Rexidine M Forte gel (Chlorhexidine, Lignocaine, and Metronidazole) to apply over the gums before brushing. 6. Povidone Iodine mouthwash twice daily. I suggest you consult a specialist, talk with them, and take the medications with their consent.

Preventive measures

1. Povidone iodine mouthwash. 2. Properly brushing and maintaining oral hygiene.
