Hello doctor,

I am 29 years old. I think I may have a fissure or tear in my perineum. It is causing some discomfort. My last sexual contact was about two months ago, and I have not been very sexually active over the past three years. I have not had any recent childbirth or surgeries.

It is completely normal for you to be concerned about anything that is likely to impact your sexual life, as it may ultimately affect your mental health. However, the distance between the posterior fourchette (posterior end) of your labia and the anus is normal.

If you have a midline tear or a fissure, that distance will be compromised, or there will be some form of muscle spasm. Additionally, a fissure or tear (perineal tear) will cause significant pain during sexual intercourse.

Mild urinary incontinence occurs in women who have carried a pregnancy to an advanced age and also in women who have had a vaginal delivery. This is because pregnancy weakens the muscles of the pelvic floor that are responsible for preventing such incidences of incontinence.

As you grow older, the incontinence may worsen. To appropriately manage it, you should start doing Kegel exercises, as this will help strengthen the pelvic floor muscles and stop the incontinence. Keep me updated about the progress of your Kegel exercises.
