Hello doctor,
I want to see what my options are based on what is said in this recent echography:
At the inferior intergluteal level, with a left orientation, a hypoechoic area is observed with a caudal extension of up to 23 mm, a depth of 8 mm, with linear echogenic content inside (probably hair follicle), and increased vascularization around to be seen in the direction of a pilonidal cyst (recurrence).

I understand your concern.
The test mentioned here seems to be pointing towards the recurrence of the pilonidal sinus (a small cyst or abscess that occurs in the skin near the tailbone (coccyx), usually at the top of the cleft between the buttocks).
So basically, the only option, as per your report, seems to be surgical intervention. The surgery includes the removal of the whole of the sinus tract along with its content, and it is left open to heal by secondary intention.
The second option is the surgical removal of the whole of the sinus tract and covering of the resultant wound with local flaps.
Some surgeons use a laser to excise the tract so that healing is better and bleeding and infection can be minimized.
Resorting to only medical treatment means without surgery, can lead to frequent recurrence as the tract may appear to heal from above (skin side) and not from below.
