Hello doctor,

My skin condition has become so severe that it is affecting my mental health and social functioning. I am 38 and have thick, inflamed plaques covering 50 percent of my body, including scalp, face, elbows, and knees. The itching is relentless, and I scratch until lesions bleed and scab.

People stare and make insensitive comments, which have destroyed my confidence completely. I had to stop attending the gym because I am mortified about others seeing my condition. Dating has become impossible due to embarrassment about physical contact.

The dermatologist prescribed topical steroids, tar treatments, and phototherapy, but nothing provides sustained improvement. My scalp psoriasis requires daily hair washing and still leaves visible flakes on clothing. Work presentations are uncomfortable because I am constantly brushing scales off my suits.

Sleep is terrible because itching worsens at night. The doctor mentioned biologic injections but warned about infection risks and high costs. Insurance denied coverage twice despite treatment failures.
