How do I manage psoriasis flare-ups and arthritis symptoms?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am dealing with thick, scaly, red patches on my elbows, knees, and scalp that are extremely itchy and often crack and bleed, causing both physical discomfort and embarrassment in social situations. These skin lesions tend to worsen during periods of high stress, and I have also noticed flare-ups becoming more frequent in the winter months when my skin is drier. Recently, I have been experiencing joint pain and stiffness in my fingers and lower back, especially in the mornings, which seems to coincide with the worsening of my skin symptoms.

My dermatologist mentioned that psoriasis can sometimes affect more than just the skin, and I am wondering if there could be a connection between my skin condition and the joint problems. Could my scaly skin patches, stress-related flares, and joint pain all be related to psoriasis, and should I be concerned about the arthritis-like symptoms I am developing?

Kindly advise.

Hello,

Welcome to icliniq.com.

I understand your concern.

Your symptoms of thick, scaly, itchy, and sometimes bleeding patches on the elbows, knees, and scalp, along with seasonal worsening in winter and flare-ups during stress, are consistent with chronic plaque psoriasis. This is a condition in which the immune system triggers an overproduction of skin cells, leading to inflammation and scaling.

The joint pain and stiffness you describe, particularly in the fingers and lower back with morning stiffness, may indicate psoriatic arthritis, a form of inflammatory arthritis that can occur in people with psoriasis. It affects the joints, tendons, and sometimes the spine. Psoriatic arthritis can develop at any stage, sometimes years after the onset of skin symptoms, and if left untreated, it can cause joint damage.

The connection between stress, flare-ups, and worsening skin and joint symptoms is well recognized, as both physical and emotional stress can intensify the immune response that drives psoriasis.

It is important to discuss your joint symptoms promptly with your dermatologist or a rheumatologist. They may recommend a physical examination, blood tests for inflammation, and imaging studies to confirm whether psoriatic arthritis is present. Early diagnosis and treatment are crucial to control both skin and joint inflammation, prevent long-term damage, and improve quality of life. Treatment options may include topical therapies, phototherapy, systemic medications, or biologic agents that target both skin and joint disease.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
