What are the safer options to maintain an erection during sex?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I think I have mild erection issues. I can get an erection during sex, but sometimes it becomes weak in between. My desire and stamina are normal, but I want to maintain firmness better. I have no major health problems. Can I take a low dose of Tadalafil (2.5 or 5 mg) occasionally, or is there a safer option?

Please guide.

Hi,

Welcome to icliniq.com.

I read your query and understand your concerns.

According to your complaints, you may be experiencing erectile dysfunction (ED).

Potential reasons for erectile dysfunction (ED) are the following:

  1. Stress: Pressures from work or family, as well as night shifts, can contribute. It is advisable to plan for regular exercise and yoga.
  2. Varicocele: An ultrasound of the scrotum can help diagnose it.
  3. Fear of Failure: Experiencing ED even once may lead to anxiety about future occurrences.
  4. Low Self-Esteem: This may be linked to chronic masturbation.
  5. Sexual Orientation: Lack of attraction to the same sex can be a factor.
  6. Substance use: Smoking and alcohol consumption can contribute to ED. Long-term illness: Conditions such as cardiac disease, thyroid disease, kidney disease, or any illness requiring long-term medication may play a role.
  7. Vitamin deficiency: Simple deficiencies can be addressed with supplements like Zincovit (a multivitamin) and Shelcal (Calcium and Vitamin D).
  8. Negative attitude towards your partner.
  9. Hormonal issues: Conditions such as thyroid problems, low testosterone, or increased prolactin levels may cause ED.
  10. Diabetes and hypertension.

I recommend you consider checking for the following tests so that we can find a probable cause for your issue:

  1. Doppler ultrasound of the penis.
  2. Ultrasound of the pelvis and scrotum.
  3. Blood pressure.
  4. Random blood sugar (RBS).
  5. Free T3 and T4 levels.
  6. Testosterone levels.
  7. Thyroid-stimulating hormone (TSH).

After doing the tests, please consult again, so that we can develop a diagnosis and treatment plan.

When it comes to enhancing our lifestyle, it is essential to focus on both physical activity and nutrition. I suggest the following lifestyle modifications for you:

  1. Exercise regularly. Aim for regular physical activity and a balanced diet.
  2. Do basic exercises like Kegels, squats, yoga, running, or brisk walking for five minutes.
  3. Try to include strawberries, apples, avocados, sweet potatoes, sesame seeds, almonds, garlic, and a small amount of chocolate in your diet.

For a temporary solution, you can use tablet Mildfil (Tadalafil) 5 mg twice a day. If you prefer to take it only before sexual activity, a dose of 20 mg should be taken approximately 30 minutes before intercourse. Please let me know if you require any additional assistance.

I hope this answers your query. Feel free to reach out anytime.

Thank you.
