I have brown spots. Can it be fungal skin infection patches?

Hi doctor,

I have a rare skin problem. It is similar to pityriasis versicolor, but it is not. In the past three years, brown spots started appearing all over my body. They do not hurt, itch, or cause any other symptoms.

I have had a skin biopsy done twice, and it revealed nothing. I was treated for fungus, but without any luck. Doctors cannot even make a diagnosis of it. It is all over my body, even on my neck and face.

I have visited several dermatologists, but nobody knows what it is. Is there any chance I can send a photo so you can better understand the problem?

I have reviewed the attached image (attachment removed to protect the patient’s identity). It appears to be hyperpigmentation post-infection (pityriasis versicolor). It will take time to fade. Here are my suggestions:

  1. Apply Ketoconazole lotion 20 minutes before bathing for 20 days.
  2. Use Candid KZ (Ketoconazole) soap for bathing for two months, or use antifungal body wash (Ketoconazole) daily for one month.
