Patient's Query

Hi doctor,

My partner and I did some dry humping with clothes, and he ejaculated but not near my vagina, and he did not pee after that. No vaginal intercourse happened in this whole scenario. After some time, we both were naked, and without protection, he started rubbing my anal area. I could not recall if he kept it correctly near my anus or the gap between the anus and vagina. But I am sure that rubbing is not near the vagina. We were in the heat of the moment, so we could not recall anything. After rubbing, he ejaculated but not near my vagina or anus. What makes me scared is, does the precum that can come in contact with my vagina make me pregnant? As precum can contain residuals of sperms from previous ejaculations, this unprotected rubbing makes me so stressed. This happened on my 15th day of the menstrual cycle.

I have irregular periods and was diagnosed with PCOS three months ago by an ultrasound scan. Before that, my period happened between 28 to 35 days of the menstrual cycle.

Six months before, I had 32 day period cycle.

Four months back, I got my period on the 12th, a 47-day cycle.

Again for the past three months, I had a 40-day cycle.

I am using a period tracker to keep my period on track, but according to that tracker, I am already late by four days.

So, I took a UPT test on the 39th day of my cycle that came out negative and a beta-hCG test in a nearby laboratory with a value of 2.24 mIU/mL.

Here are my doubts-

1. Is there a possibility that the precum can drip down to my vagina from the anus and gets me pregnant though we had no vaginal or anal intercourse?

2. I have taken two tests, UPT and beta-hCG. Are they accurate? Or did I take the tests too early for a pregnancy to be detected?

3. On the day of sexual intimacy, I had tender breasts, which I later came to know that is my ovulation day which is an added stress.

4. The same situation happened four months back since I skipped my period due to PCOS, which I was unaware of before taking a beta-hCG test. After, I missed my period by seven days, and the value showed 0.003 mIU/ml. And why the HCG increased from 0.003 to 2 in just two months. Is it because I am pregnant this month and took the test too early? Or any other reason?

5. Can the value of 2.24 be doubled in my case?

6. We did not have any vaginal intercourse till now. Only dry-humping has been happening since we decided to take this relationship further.

To induce my period four months back, I took Medroxyprogesterone five days straight, and after two days of completion of the drug, I got my period. And two months back, I did not take the medication to get a period; it came on its own.

I am currently waiting for my period to come. Today is the 40th day of my cycle. Am I pregnant? Are the tests accurate, or should I repeat the tests after some days? I did not use any emergency contraceptive or birth control pills after this incident.

Answered by Dr. Reetika

Hello,

Welcome to icliniq.com.

I have read the details presented by you and also gone through the report shared by you (attachments removed to protect the patient's identity). With the details given by you, the pregnancy tests are already done by you (urine and beta human chorionic gonadotropin (HCG) test), and I suggest that you are not pregnant.

I am giving answers to your queries:-

1. Sperm ejaculate or precum contains a much lesser quantity of sperms; in both cases, sperms can barely survive for a few seconds after being released, and there is no chance they could have dripped down, reached your vagina, and then entered inside.

2. You did both the tests yesterday, that is fine, and after 25 days of intimacy, both can be considered accurate.

3. I suggest you not to worry about ovulation, as the sperm never reached your vagina, so the release of the egg (ovulation) is not a concern.

4. Any value of beta-hCG less than 3 is non-pregnant; it is primarily varying according to the accuracy of the lab and kits and also when in your cycle you took the test.

5. No, it would not double; it have already been the third week if you had conceived.

6. Dry humping or intercourse with a birth control pill or condom is safe.

Now I suggest you avoid stress as stress causes a delay in periods in 25 % females worldwide. So you have a history of irregular periods, and this is a repetition of that, and do not worry about pregnancy.

Feel free to ask any questions or to share any additional information.

Patient's Query

Hi doctor,

Thank you so much. I was under so much stress for the past week. I was like not eating and sleeping on time, even though my partner had been by my side and reassuring me. I do not know, but I have a fear of getting pregnant. Even though I will be getting married in a few years, if I came to know I am pregnant, I cannot handle it. I had a reason for being terrified because my mom had three miscarriages after me, and the pain she felt was unbearable. My mom had my twin brother and me in her womb during her first pregnancy. Unfortunately, my twin brother died in her womb and she had to deliver me prematurely in the 7th month. After some years, she had back-to-back miscarriages as my parents wanted to try for another baby so badly, and I, too, wanted a sibling. And all the three miscarriages she had were twins only. From then on, I have had this fear of being pregnant. Whenever I hear that someone is pregnant, I would get so panicked. I do not know how I can overcome this fear.

And thank you once again; I will be relieved and wait for my period to come. And I have another doubt too.

1. How can I track my irregular periods?

2. What are ovulation tests? How do they work? And which ovulation test kit would work best?

3. For emergency contraceptives, which pill would you like to recommend in any emergency cases so that I could keep that with me?

4. And also about birth control pills, what is your recommendation?

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

Glad to hear that I can help you. Here are my suggestions:

1. Multiple applications are available to help track periods; you may use any depending on which one you like. All are more or less similar.

2. There is no need for you to track ovulation; at this stage, focus on your studies or career. It is more relevant for females planning pregnancy.

3. Rather than emergency contraception pills (ECP), please insist on your partner carrying a condom because ECP will interfere with your menstrual cycle and ovulation. If in crisis, use a tablet I-pill (Levonorgestrel).

4. Since you are not that active sexually, once in a month type, please do not start using the birth control pill. Instead, tell your partner to use a condom. It will protect you from unplanned pregnancy and also sexually transmitted diseases.

I hope it helps.

Patient's Query

Hi doctor,

Thank you for your reply.

I have some other doubts-

1. What are the chances of getting pregnant by precum during ovulation and non-ovulation days?

2. Precum can have residuals of sperms from previous ejaculations, right? In my case, my partner ejaculated minutes before this unprotected rubbing. What if his penis got in contact very close to my vaginal hole instead of my anal area? Are the tests I took still considered valid and accurate?

3. What if I did not ovulate on the day of sexual intimacy, or if I ovulated after intimacy or before intimacy? Can pregnancy still happen? Are these tests considered accurate?

4. In the case of irregular periods, when should we take a pregnancy test if the cycle lasted as 40 to 47 days?

5. Can taking tests after three weeks of sexual intimacy be considered accurate even when we do not know when the ovulation happened or even if there is the involvement of sperm but not precum?

6. When the egg is released, how long will it be alive before it breaks?

7. How many days does it take for fertilization and implantation bleeding?

8. When should we consider safe days even the condoms are used? And how many days are considered fertile days?

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

I have suggested you follow the below instructions-

1. For preventing unplanned pregnancy, assume the chance of pregnancy to be 100 % with precum during ovulation.

2. Yes, the tests are valid. On the lighter side, it is sperm only, not the targetted rocket.

3. Take a minimum of three days, before or after ovulation to prevent unplanned pregnancy.

4. After 18 to 20 days of intercourse, it is fine to take the test in case of irregular periods.

5. Yes, with five days of sperm life and then 14 days, so it is accurate if you do a pregnancy test after 21 days.

6. No specific answer can be given on this as the result of an unfertilized egg not being implanted is called periods.

7. Around 10 to 15 days.

8. No days are safe for staying from unplanned pregnancy. However, 9 to 21 days is a high-risk period for fertile days. I will rather put it this way for you.

I hope it helps. Take care.

Patient's Query

Hi doctor,

Thank you for your reply.

I have had moderate pelvic pain since morning, and the pain is getting a little heavy when I am urinating. When I touch it, I feel like there is bone (I do not know what the inside part is, but it is like a hard one), and it is painful when touching. And also have backache, feeling so much bloated since I had my dinner, and stomach stiffness. Is this a problem?

Kindly help me.

Hello,

Welcome back to icliniq.com.

This is not a major problem unless there is an unusual discharge from the vagina, bad smell, or burning sensation during urination.

Take care.

Patient's Query

Thank you doctor for the reply,

I have consulted a gynecologist in my city. She said to take a scan because I had pelvic and lower abdomen pain even after my period ended. I was about to consult her. I will be attaching my scan report here. According to the report, is there any sign of a yolk sac? I had been bleeding for a week. Should I consider it as my period? Or is that any implantation bleeding? Should I repeat my UPT after some days, or can I be relieved that I am not pregnant?

Thank you.

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

There is no evidence of pregnancy from the attached images (attachment removed to protect the patient's identity). No need to repeat the urine pregnancy test.

I hope this helps.

Patient's Query

Hello doctor,

One month back, my partner and I did some dry-humping with clothes on. I wore three layers of clothing leggings, shorts, and undies. My partner wore only shorts. His penis got out of his shorts, so he ejaculated on my leggings near the elastic band of the waist. I cleaned it with tissues. As far as I remember, there was no visible cum after cleaning with tissues. We hugged each other for five to seven minutes, and after that, he fingered me. While fingering, I thought, what if his hands had any cum? So, I removed my pants just in case. After that, I went to wash, and there was no visible cum but dampness near the ejaculated area.

I had PCOS and irregular cycles. However, my cycles have been regular for the last two months. This month, my period was more scanty than a heavy flow with red and brown clots. Today, I went for an annual check-up, and the doctors performed a scan.

I have a few questions:

1. Is there a chance of pregnancy?

2. Uterus measuring 76 x 28 x 32 mm. Anteverted. Uniform myometrial echotexture. No focal lesions.

Endometrial thickness measuring 6 mm. The right ovary measures 26 x 14 mm largest follicle noted measures 5 mm. The left ovary measures 27 x 12 mm largest follicle measures 6 mm. What does this mean?

3. What is the meaning of the largest follicles? Will the values remain like that without any change? Does that mean any pregnancy? Will they vary from time to time during the entire cycle? Or will they be stable? Are the values normal ?? or is there any indication?

My doctor has prescribed the tablet Dronis 30 (21 pills) three strips for three months. She said to start the pills today. However, I am using tablet Paracetamol and Tendoforce as I have muscle pain. Will the efficiency of the oral contraceptive pills decrease if I use them with the other medicines? Also, how do I use the oral contraceptive pills? When should I start and stop taking the pills, and how much gap should I have? Could you please give me directions? I will be attaching my scan report that was taken today.

Please help me.

Thank you.

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

I have gone through the details presented by you. There are no chances of pregnancy as the sperms survive for only a few seconds in an open environment after coming out of the penis. The pollutants and particles in the air and the skin layering do not allow the sperm to live for long. The test results are accurate, and you are not pregnant.

Just stay stress-free, and your periods shall be there anytime now.

Kindly follow up for further queries.

I hope this helps.

Patient's Query

Thank you, doctor,

We did not have any sexual activity until two months before this, and I also had my periods. What I am worried about is, what if he touched an ejaculated part near my pants and fingered me? While he was fingering, I wanted to take off my leggings to avoid contact with sperm near the ejaculation part. Is there any chance of pregnancy from this, as he touched the fingered me? As it was my starting days of fertile days, we avoided unprotected intimacy at all costs. There was not even direct genital-to-genital contact. I am worried about what if his hands had sperm, which could make me pregnant.

Please help.

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

I have gone through the details presented by you and I suggest you do a UPT (urine pregnancy test) just to put your mind at ease. Further your endometrial thickness was not suitable for pregnancy. It was very thin. There is nothing to worry as you are perfectly fine. I hope you have started the medicine prescribed by your doctor.

I hope this helps.

Take care.

Patient's Query

Hello doctor,

Thank you for your reply.

I took the beta hCG test a few days back, which was negative. And also, I had another scenario where I need your help.

From the seventh day of my cycle, I started taking the tablet dronis 30 as I had spotting before that. On the 11th day after my cycle, my partner fingered me. I am not sure if he had touched his penis or not. My questions are:

1. What if he touched his penis and fingered me? Is there a chance of pregnancy? And also, by taking the tablet Dronis 30, can we say that ovulation has not happened on that day?

2. On the seventh day of my cycle, when I did a scan, there were two follicles in both ovaries measuring 5 mm and 6 mm, respectively. So do you think ovulation will happen soon after on the 11th day of the cycle, or will ovulation not happen at all?

3. A few days later, I had back pain and lower abdominal pain, which was mild but uncomfortable. My vaginal discharge was thick and creamy. Was it because of any implantation or ovulation?

4. In this whole scenario, we have not removed our clothes. He slid his fingers and rubbed my clitoris.

5. My tablet Dronis 30 pack will end in a few days. When can I take a beta hCG test?

Please advise.

Answered by Dr. Reetika

Hello,

Welcome back to iclinq.com.

I hope you are doing well.

The answers to your queries are as follows:

1. There is no chance of pregnancy as sperms do not survive in the open for long due to chemicals, pollutants, etc, in the air.

2. It cannot be told precisely regarding ovulation based on your shared information. There are multiple other factors.

3. The discharge is most likely due to ovulation.

4. There is no need to worry about pregnancy.

5. You can take a pregnancy test after three days.

I hope this helps.

Take care.

Patient's Query

Hello doctor,

Thank you for your reply.

I will attach some pictures for your reference. Please see the pictures and suggest whether it is an ovulation discharge. If the tablet Dronis 30 is a contraceptive pill, how does it prevent ovulation? Is my body not responding to the medicine, or should I change the pills? Is day seven too early to stop ovulation? Will it work better if I take from day 10?

Please help.

Answered by Dr. Reetika

Hello,

Welcome back to icliniq.com.

Yes, it is most likely ovulation discharge. Are you having any itching or foul smell near the vagina? Do not change medication frequently. Have trust in your treating doctor. Do not take the stress and focus on routine things of life with positivity.

I hope this helps.

Take care.

Patient's Query

Hi doctor,

Thank you for your reply.

However, I have a few questions,

1. If ovulation happens, the cervical mucus or discharge would be egg white, thick, and stretchy, right? But my discharges are all white, creamy, and thick. Is that ovulation discharge or my normal discharge?

2. I have been taking the tablet Dronis 30 from day seven of my cycle, and as you said, ovulation would not occur so early, and it would stop ovulation, right? And you said the vaginal discharges are most likely due to ovulation. But there is no egg white discharge in my case.

3. All the pills I have taken since starting are taken on time every day at 10:00 PM without delay, not even a minute late. Do you think ovulation occurred when I was taking the pill?

4. As I take the tablet Dronis, I stopped the last two pills because I have had a severe headache, likely due to stress. I did not take the last two pills for two days. And from yesterday, I started having brown bleeding, which is lighter than the one I had last month, with clots and cramps.

5. Is this bleeding happening because I stopped taking the pills or implantation bleeding? This is exactly what I experienced during last month's bleeding. I have not had a proper period for the last two months. This is making me even more stressed.

6. And also about the pills, I wanted to ask you one thing, As I had stopped taking the last two pills, will ovulation happen if pills are stopped mid-pack? Or will it cause bleeding? As of now, I stopped taking the last two pills, and I have been having some brown bleeding. Is this ovulation bleeding or implantation, or withdrawal? I am confused. I do not know what to believe. I am so stressed.

7. And there are other reasons why I have been this stressed. They say late ovulation causes late periods. And also, late ovulation can make a pregnancy go unnoticed or, more likely, it will take time to get a positive result. I do not know if I take a test and if it is negative, I would believe that it was a false negative and that I was pregnant.

I know I am being paranoid. And thank you for patiently answering my questions. I am just really stressed. This is the reason why I am avoiding sexual intercourse with my partner, and now without intercourse, also I am being paranoid about pregnancy. I do not know what to do anymore.

Please help.

Patient's Query

Hello doctor,

I am relieved that there is no chance of pregnancy. Also, my partner said that there was no sperm on his finger. On the fifth day of my period, I started getting severe pain on the right side of my back, pain during urination, and frequent urination. My gynecologist sent my urine sample for testing. I had no infection in my urine. Also, they did a scan to rule out kidney stones.

My reports read as below:

  • A well-defined anechoic area measuring 6x4 mm noted in the posterior lip of cervix in the lower uterine segment. It shows a peripheral rim of echogenicity.
  • Non-obstructive right renal calculus.
  • Polycystic morphology of right ovary.
  • Early gestational sac with decidual reaction or a nabothian cyst?
  • Suggest Beta hCG and serial ultrasound scan.

Kindly explain.
