My wife is taking Ecospirin during her third trimester. Should she discontinue it?

Hello doctor,

My wife is in the third trimester of pregnancy (30 weeks). Since 20 weeks of pregnancy, her gynecologist has advised her to take Ecosprin 75. She has been regularly taking it since then. Her platelet count in the first trimester was 1.5 lakhs.

However, in the third trimester, it reduces to 0.98 lakhs. She is 32 years old, and this will be the second issue. The previous delivery was four years back through a C-section because the fetus was in a breech position. Except that there was no problem in the previous pregnancy.

She has no history of hypertension, diabetes, or renal problems, and her BMI is 28. Can she discontinue Ecosprin 75? Or what else she can do to increase the platelets and prevent any complications at the time of delivery and beyond.

Apart from low platelets (adequate on smear), everything seems to be normal. If you are not hypertensive, have no previous abortions, or fetal growth restriction, then why were you put on Ecosprin? You can discuss this with your doctor regarding Ecosprin initiation, as there must be some reason for that.

So I will advise not to discontinue without discussion with your doctor. Low platelets can be due to vitamin B12 deficiency, hypothyroidism, or dehydration. Sometimes, some patients have gestational thrombocytopenia without any reason. Get vitamin B12 and D levels done. You can take citrus fruits like kiwi to increase platelets. Take plenty of oral fluids and have a balanced diet.
