Patient's Query
Hello doctor,
I am concerned about a few related sexual health issues and would like your suggestions for the following:
I experience nightfall, or involuntary discharge of semen during sleep, about once a year. Is this normal, and should I be concerned?
During sexual activity, I tend to ejaculate very quickly, usually within a minute of penetration, and feel I have little control.
I am also concerned that my penis size may be smaller than average. I would appreciate your professional opinion on what is considered normal and whether any evaluation is necessary.
Thank you for your time and guidance.
Hello,
Welcome to icliniq.com.
I understand your concern regarding your sexual health.
Experiencing a nocturnal emission (nightfall) once a year is not a cause for concern; this is completely normal and does not indicate any medical problem.
As you mentioned, you ejaculate within one minute of penetration. This condition is known as premature ejaculation (PE). It can occur due to factors such as performance anxiety, penile hypersensitivity, or neurochemical factors (like 5-HT receptor imbalance). Please do not worry; it is a common and treatable condition.
Treatment includes: Initial management focusing on psychological and behavioral techniques. Medication is considered only if these methods do not provide sufficient relief.
Behavioral or psychological techniques that help you:
A start–stop technique can be done when you feel close to ejaculation. Stop stimulation, wait until the urge subsides, and then resume.
You can follow the squeeze technique at the point of near climax: gently squeeze the glans (head of the penis) for a few seconds to reduce arousal, then continue.
With the sensate focus technique during intercourse, shift your attention away from penetration; for example, focus on touching, kissing, or other sensual activities. You can also try to stay relaxed by engaging in light distractions like background music or conversation.
Practicing these regularly helps you gain better control over ejaculation.
If symptoms persist after trying these methods, I can suggest appropriate medications to further help you. Do not worry; this condition is very manageable, and you will definitely see improvement.
Please feel free to follow up with any further questions or updates. I will be happy to assist you.
I hope this helps.
Thank you.
